Subchapter 02T, Section.0405,to request the submittal of a collection system permit application for the
area serviced by the wastewater treatment system referenced above. The application should be received
within 180 days of receipt of this notice. The application can be downloaded using the following steps:
• Go to: https://deq.nc.gov/about/divisions/water-resources/water-resources-permits/wastewater-

• Click on Wastewater Collection System Application—New/Renewal(Form CSA 04/16)
The original application, signed by an authorized signing official,with requisite supporting
documentation as well as one digital copy of the complete application packet must be submitted to the
