Steffens, PE 2301 Rexwoods Drive, Ste 200 Raleigh, NC 27607 Subject: REQUEST FOR ADDITIONAL INFORMATION Blenheim Stream Restoration.& Culvert Repair Dear Ms. Steffens: Donald R. van der Vaart Secretary DWR # 15 -0585 V2 Wake County On September 18, 2015, the Division of Water Resources (Division) received your application requesting a 401 Water Quality Certification / Buffer Authorization from the Division for the subject project. The Division has determined that your application is incomplete and cannot be processed. The application is on -hold until all of the following information is received: 1. No Check Included A review of the Pre - Construction Notification (PCN) Form indicates this application is for _177_ linear feet of stream impacts. Pursuant to 143- 215.3D (e), a $570 permitting fee is required to process this application. Please remit a check to DENR -DWR, 401 & Buffer Permitting Unit, 1617 Mail Service Center, Raleigh, NC 27699 -1617. 2. Need more copies of the application (401 WQC and Isolated) Pursuant to 15A NCAC 02H .0502 and G.S. 143- 215.3D(e), the Division requires four (4) complete application packages, to begin the proper consideration of an application. We received 1 copy of the application, however we require 4 copies of the application. Please submit 3 additional copies of the application. Pursuant to Title 15A NCAC 02H .0502(e) the applicant shall furnish all of the above requested information for the proper consideration of the application. If all of the requested information is not received in writing at the address below within 30 calendar days of receipt of this letter, the Division will be unable to process the application and it will be returned. The return of this project will necessitate reapplication to the Division for approval, including a complete application package and the appropriate fee.
