line ditch that is located between his two properties which are titled in H & R Investments of
Camden, LLC and Eastern Carolina Construction, Inc. to determine if relocation is feasible.

2. A scaled copy of a portion of the USGS quad map for the site with the site identified
3. An enlarged copy of the USGS quad map area to better show the site boundary.

5e. Provide a brief description of this project (attach site plan if available): Property owner would like to reroute the existing
ditch to eliminate the division between the two tracts of land he owns. The goal will be to relocate to the NCDOT roadside
ditch location and connect to the existing ditch (also shown as blue line) on the western property line. Appropriate permit
applications will be filed if it is determined that the ditch is not considered a stream.
Please attach a map of the site indicating project boundaries on the USGS 1:24,000 Topo and /or NRCS Soil Survey.
If you are unable to locate either of these maps, please contact the Regional Office for assistance.
