The Annual Permit Certification must be completed each year on or before the day of permit issuance, October 12. Failure to do so is a violation of your
permitting conditions and is subject to enforcement action per G.S. 143-215.6.
As indicated in the Annual Certification Form submitted on January 30,2024,the project is not in conformance with all of the permitting conditions and
must be brought into conformance via the corrective action and timeline provided in Section E.of the Annual Certification Form.
