Dee Freeman Governor Director Secretary January 25, 2012 George Thompson 1046 Whirl-Win Drive Winston-Salem, NC 27107 Subject: Compliance Evaluation Inspection NPDES General Wastewater Permit NCG550000 Certificate of Coverage: NCG550057 1046 Whirl-Win Drive, Winston-Salem Forsyth County Dear Mr. Thompson: Ron Boone, of the NC Division of Water Quality (DWQ), Winston Salem Regional Office (WSRO), conducted a compliance evaluation inspection of your single family residence wastewater treatment system located at 1046 Whirl-Win Drive, in Winston-Salem, Forsyth County, NC, on January 24, 2012. Your assistance and cooperation during the inspection was greatly helpful and appreciated. The findings of the inspection are summarized below and an inspection report is attached for your records. Your single family residence wastewater treatment system is regulated under NC NPDES General Wastewater Permit NCG550000 and has been issued certificate of coverage (COC) number NCG550057. The system discharges to Lowery Mill Creek, which is currently classified as Class WS-III (water supply) waters and is located in the Yadkin Pee Dee river basin. Your system consists of a septic tank, a filter field, and a chlorinator. You were able to show Mr. Boone the approximate locations of the tank and filter. There was no evidence of failure of any of those components. According to you the septic tank was pumped sometime within the past five years but no documentation was immediately available during the inspection. There are currently two people occupying the home. Please note that the general permit requires you to have the tank pumped once every three to five years. The chlorinator was found and inspected. The tablets you currently use for disinfection are rated for use with swimming pools, however you should use tablets rated for use with wastewater. You may be able to find this type of tablet at local plumbing supply businesses or on the internet. A listing of potential sources is also attached to this letter. The pipe is made of non-rigid black 4" corrugated plastic pipe and the end of the pipe was hanging down into the stream. You should cut the end of the pipe off or support it somehow so that the end of it does not hang into the stream. Mr. Boone was unable to determine whether there was a discharge at the time of the inspection because of this. However, he reports that the stream looked clean and healthy and there appeared to be no adverse effects of the discharge on the stream. Samples of the discharge have not been taken. Please note it is also a requirement of the general permit to have the discharge tested annually. If the opportunity arises you should take samples of the effluent and have them tested by a local NC certified laboratory for those parameters specified in the general permit. A copy of the general permit, as well as a listing of NC certified laboratories are also attached to this letter. Your permit was recently inactivated due to failure to pay the annual fees for 2011. Charles Weaver, of our Central Office in Raleigh recently mailed another invoice and appropriate paperwork out to you to get the fees paid and reinstate the permit. Please pay the appropriate fees and submit all paperwork as requested by Mr. Weaver as soon as possible. Lastly, you are encouraged to keep all paperwork relating to the wastewater system in a single folder in a safe place. This would include all correspondence sent to you by the Division, records of having the tank pumped, effluent testing records, etc. This will not only simplify the inspection process in the future but also provide you good records of the home's wastewater system if you should ever decide to sell the home. Apparently her husband has dementia or something and gave me some false information. Anyway, I told her I'd have you guys send out another renewal notice. May want to also send her the name change form to have the stuff put into her name. Her name is Linda Thompson. Not sure about the status of the foreclosure but she wants to renew it. What say you?
