On the NOT Certification Form, the legally responsible organizational entity should be Ecoplexus Inc. and the legally
responsible person should be Roger Baele. Either have Roger Baele sign the NOT Certification Form and submit the
eNOT, or a delegated individual from the Ecoplexus Inc. to print their name and title on the "Print Name and Title of
Signed if Authorized" line of the NOT Certification Form, sign the form using an original wet signature, and upload the
updated NOT Certification form.
