Yes X No Specific to Spill/Bypass or Routine Sampling: Sampling was done after spill was discovered to have entered surface waters. (Initial observation suggested spill had only made it to edge of the woods) Upset, Spill, or Bypass 5-Day Reporting Form Page 2 Source of the Upset/Spill/Bypass (Location or Treatment Unit): A underground pipe from first stage aeration basin leading to a manhole that ties into the Influent pump station #1 Cause or Reason for the Upset/Spill/Bypass: Pipe was broken underground at a manhole Describe the Repairs Made or Actions Taken: No repairs have been made to the pipe as of right now. The valve from the aeration basin will remain turned OFF. Action Taken to Contain Spill, Clean Up and Remediate the Site (if applicable): Found the valve to shut off flow to manhole. Scraped sludge in piles and got the vac truck to come suck up the sludge and put it in the drying bed, put lime down on the ground. Got a contractor to mobilizing with 30 to 40 personnel to clean up the sludge up in the wood line to the creek. Action Taken or Proposed to be Taken to Prevent Occurrences: Going to repair the line from the aeration basin at the manhole. And going to leave the valve OFF to that line. This line is not necessary for general processing. Additional Comments About the Event: We thought the spill was coming from a new force main from the Influent pump station #1. So we were truing to isolate the force main from the pump station to the splitter box wing to stop the spill. In the process of isolating the force main we took the lid off the manhole and put sand baps down around where the spill was coming up the side of the manhole and diverted it into the manhole. Realizing the force main was not the problem we first excavated around the spill area and traced the source that followed a line entering the manhole and then looked at the pipe drawings of the plant and found a valve to shut the flow off heading to the manhole. Upstream, Downstream, and at Ground Zero. Daily up to date emails will be sent every morning on the cleanup.
