Per G.S. 143-215.1 C(b), the owner or operator of any wastewater collection system shall:
In the event of a discharge of 1,000 gallons or more of untreated wastewater to the surface waters of the State, issue
a press release to all print and electronic news media that provide general coverage in the county where the
discharge occurred setting out the details of the discharge. The press release shall be issued within 24 hours after
the owner or operator has determined that the discharge has reached surface waters of the State.
In the event of a discharge of 15,000 gallons or more of untreated wastewater to the surface waters of the State,
publish a notice of the discharge in a newspaper having general circulation in the county in which the discharge
occurs and in each county downstream from the point of discharge that is significantly affected by the discharge. The
Regional Office shall determine which counties are significantly affected by the discharge and shall approve the form
and content of the notice and the newspapers in which the notice is published.

Describe the "severe natural condition" in detail: Winter Storm Finn caused the Town to receive heavy rainfall in excess of
2 inches within a 12 hour period.

Explain if Yes: The Town has had 3 sewer rehab project since 2012 to repair the system, projects are currently ongoing.
What corrective actions have been taken to reduce or eliminate I & I related overflows this spill location
within the last year? Approximately 14,300 LF of gravity sewer has been video inspected and point repairs have
been identified and corrected under the NC CWSRF project CS370698-01 to the extent allowable under the
funding. The video inspection did not identify needed point repairs to the Cokey Swamp Interceptor at the
location of the SSO and it appears the source is above stream of the video inspection.

Comments: Some of the repairs identified by the video inspection require replacing the lines and we are seeking additional
funding to perform these follow up line replacements. In addition, we are beginning work on a new project -2023 CWSRF
Wastewater System Improvements Pump Station & WWCS Improvements (RP-W-0219) which will provide additional
video inspection and point repairs. Unfortunately, the focus of the I&I portion of that project is another problem area in
Town, the Brickyard Pump Station which does not contribute to this SSO. We are planning to make further investigation
because of this SSO to see if some of those funds should be redirected to the Cokey Swamp Interceptor, Hilltop Drive
Area, and Sandy Knob Lane area which appear to be the source of this I&I. Comprehensive flow study may be needed to
further identify the root causes. We may apply for funding to accomplish additional I&I work to reduce the extraneous
flows contributing to the Cokey Swamp Interceptor.
