A. Attachment A — Site Map:
1. The permit description includes two 1,000 gallon septic tanks in parallel - one for gray water and
one for sanitary wastewater, a 150 square foot subsurface sand filter for gray water, a 255 square
foot subsurface sand filter for sanitary wastewater, and two 1,000 gallon storage/pump tanks.
During a compliance evaluation inspection with Winston-Salem Regional Office staff, MT.
Waggoner stated he was not aware of the second septic tank, but was aware of the second sand
filter. Please locate the second tank and confirm whether the gray water parts of the septic system
are still in use.
B. Attachment D — Easement, Encroachment, and Lease Agreements:
1. The spray irrigation area appears to be located on the neighboring parcel (Parcel ID: 8894182116),
owned by DLT Holdings, LLC. Please either provide a copy of an easement agreement. This
agreement must be notarized, signed, and recorded with the Alamance County Register of Deeds.
If the spray irrigation field is not located on the neighboring parcel, please provide a drawing
