The above project qualifies for Nationwide Permit 3 authorization. This permit authorizes replacement of an
existing culvert structure under NCSR 1374 (Mt. Herman Church Road) in an unnamed tributary to Little River near
Elizabeth City, Pasquotank County, North Carolina. The existing 30' by 45' pipe and 24" by 45' pipe will be replaced with
a 95" by 67" by 40' pipe. There will be no permanent impacts to jurisdictional waters or wetlands, however, a temporary
sheet pile cofferdam will be used to dewater the project area while the culvert structure is being replaced. Any
modification of the project that causes additional impacts to jurisdictional areas may require authorization from this
office prior to initiation of work.

Please see the attached authorization and conditions for NWP #3. In addition, see the following Special Condition:

- Cofferdam and associated dewatering structures must be removed in their entirety upon project completion.
