3999 NC Highway 88 West

Dear Ms. Medley:
On December 12, 2023, Division of Water Resources (DWR) Winston-Salem Regional Office (WSRO) staff Jesse
Barnes conducted a routine compliance evaluation inspection of the single family residence wastewater treatment
system located at 3999 NC Highway 88 W, Warrensville, NC 28693, in reference to NPDES general permit
NCG550000 and certificate of coverage NCG550500. Ms. Mae Medley, owner, was present during the inspection. A
review of the subject permit, records, treatment components, outfall area, operations, and maintenance was
completed. This review reflected noncompliance with the subject permit. The determined permit violations are
listed below. Inspection findings are detailed in the attached Water Compliance Inspection Report.
The following violation(s) were noted during the inspection:
Inspection Area Description of Violation
Effluent Sampling Effluent monitoring has not been conducted. Part I, Section C. (1.) of the subject permit
requires the discharge to be monitored and analyzed annually for the listed parameters.
Disinfection There is no known means of disinfection for the system. Part I, Section B. (1.) requires
systems existing prior to August 1, 2007 to include means of disinfection.
The indicated permit violation(s) must be corrected to achieve compliance with the subject permit. Please be aware
that the Division may take enforcement action against any individual who fails to comply with the requirements of
all applicable permits, regulations, and/or statutes. Pursuant to NC General Statute (NCGS) 143-215.6A, a civil
penalty of not more than twenty-five thousand dollars ($25,000.00) may be assessed against any person who
violates or fails to act in accordance with the terms, conditions, or requirements of any permit, or applicable
regulations, issued pursuant to G.S. 143-215.1.

Permit Violations

existing prior to August 1, 2007 to include means of disinfection.
Other observations:
Ms. Medley stated that she has not had any problems with the system since the last inspection. If
chlorine tablet tubes are/were present, either their location is unknown or they have been removed -
effluent wastewater is not undergoing disinfection. Ms. Medley stated that the septic tank is pumped
once every five years and it was last pumped July 25, 2023. Records for septic tank pumping were
available for review during the inspection. The outfall was easily accessible and observed during the
inspection. No discharge was observed during the inspection and the residents stated that they have
never observed a discharge. The receiving stream was free of floating solids and foam.
