Dear Ms. Robbins:
On December 12, 2023, Division of Water Resources (DWR) Winston-Salem Regional Office (WSRO) staff Jesse
Barnes conducted a routine compliance evaluation inspection of the single family residence wastewater treatment
system located at 2634 Old Highway 16, Jefferson, NC 28640, in reference to NPDES general permit NCG550000
and certificate of coverage NCG550478. Ms. Jane Robbins, owner, was present during the inspection. A review of
the subject permit, records, treatment components, outfall area, operations, and maintenance was completed. This
review reflected noncompliance with the subject permit. The determined permit violations and items of concern are
listed below. Inspection findings are detailed in the attached Water Compliance Inspection Report.
The following violation(s) were noted during the inspection:
Inspection Area
Description of Violation
Effluent Sampling
Effluent monitoring has not been conducted. Part I, Section C. (1.) of the subject permit
requires the discharge to be monitored and analyzed annually for the listed parameters.
Disinfection
There is no known means of disinfection for the system. Part I, Section B. (1.) requires
systems existing prior to August 1, 2007 to include means of disinfection.
In addition, the following item(s) of concern were noted during the inspection:

Recordkeeping
No records for septic tank pumping were available during the inspection. Ms. Robbins stated
that it is pumped once every five years and was last pumped approximately two years ago.
Effluent Pipe
The exact location of the outfall pipe is unknown and was not located during the inspection.
The indicated permit violation(s) must be corrected to achieve compliance with the subject permit. Measures should

be taken to address the indicated item(s) of concern. Please be aware that the Division may take enforcement
action against any individual who fails to comply with the requirements of all applicable permits, regulations, and/or
statutes. Pursuant to NC General Statute (NCGS) 143-215.6A, a civil penalty of not more than twenty-five thousand
dollars ($25,000.00) may be assessed against any person who violates or fails to act in accordance with the terms,
conditions, or requirements of any permit, or applicable regulations, issued pursuant to G.S. 143-215.1.

Other observations:
Ms. Robbins stated that she has not had any problems with the system since the last inspection. If
chlorine tablet tubes are/were present, either their location is unknown or they have been removed -
effluent wastewater is not undergoing disinfection. Mr. Barnes searched the property and receiving
stream bank for chlorine tablet tubes and an outfall pipe, but neither were located. The receiving
stream was free of floating solids and foam.
