I have reviewed the second version you submiƩed and there seems to be some conflicƟng informaƟon.
The FTA 06-21 Form SecƟon VI. 7 and 8:
7. You state 250 gal/ day for 128 equals 32,000 gpd. But your total has 32,500 gpd.
8. Tou state that 65,000 gpd is generated. This number should match the above number.

In your email dated November 27, 2023, you stated there are 262 units for a total of 65,500 gpd. Can you clarify which
number is correct. I need to make sure the permit has the correct values.

Please review the following comments in order for you fast track to proceed. Your Addi Ɵonal InformaƟon should be
received within thirty (30) days or you will need to resubmit your package again.

15A NCAC 02T .0107 Staff Review and Permit Preparation states:
 “(2) (e) If an application is accepted and later found to be incomplete, the applicant shall be advised how the
application or accompanying supporting information may be modified to make it complete. The staff shall advise
the applicant:
(2) if all required information is not submitted within 30 days, the project will be returned as incomplete.
Any resubmittal of a returned application shall be accompanied with a new application fee.”

The following discrepancies were noted in the review of the Whispering Pines Phase 4-5 review.
1. Form FTSE 04-16: Please have #1 completely filed out. We need to see that the WWTP is not reaching
capacity. Please fill out or have Chris Doherty fill out 1.d, 1.e, 1.g, and 1.h.
2. Also please add Facility Name and Permit Number at the boƩom of the page.

Please note, I will not be in next week and will return Jan 3, 2024.
