In the meantime, I noticed that for some reason the facility address is not listed correctly in the database. See attached
Owner/ Permit summary forms checked on today plus the change in responsible person form submitted early this year.
The facility address and permit owner address are the same and should both be listed as:
313 Crescent St NE
Valdese, NC 28690
The Permit Owner Responsible Official is correct but not sure why her title is abbreviated as the paperwork originally
submitted listed General Manager.

Resp. Official Title: GM
Please update title to General Manager.
