Henderson, NC 27537 T:252-492-1131 F:252-492-3531 In accordance with permit requirements, Ardagh Glass Inc. is submitting this 5-Day discharge report. On December 8, 2023 at approximately 6:30AM Randy Bishop, Plant Engineer received notification of an operation issue related to the Closed -Loop Recycling System. An investigation revealed that a pump control device and a secondary check valve failed resulting in a discharge of approximately 5,000 — 10,000 gallons of contact process water. The process water is used as a coolant for production equipment and is exposed to lubricating petroleum products. A float switch malfunctioned on one pump along with the secondary check valve on a second pump. This resulted in the one pump that was operating to just keep pumping the process water back into the sump pit instead of out to the tank. This caused the sump pit to overflow. The process water flowed out onto the yard and into a storm drain located on the southeast side of the facility. The storm drain empties out to Facility Outfall #1. An inspection of Outfall #1 was conducted approximately 3 hours after the received notification of the incident. The appearance of the water surface indicated that the oily material was contained in the outfall. There was no oily sheen evident on the water leaving the outfall. Shamrock Environmental was on -site at about 2PM that afternoon and pumped out the outfall. At that time there was still no visual evidence of oily material leaving the outfall. The corrective actions that are being considered to prevent a recurrence include; control device redundancy, alternative control devices and alarm system modifications.
