Dear Mr.Jaekel:
A review of the Division's billing records indicates that your Annual Administering and Compliance Fees
for the above-referenced permit have not been paid. This fee requirement is documented in your currently
active permit, Part III Section B. 14 [NPDES Permit Standard Conditions Version 11/09/2011.1,p. 11].
Your annual fees are due on the anniversary of your original permit effective date(December 3, 1996).
There was an error in our database that resulted in no annual fee invoices being generated and mailed
since the year 2005. The Division is waiving fees for the years 2006 to 2011 (45,160). Thus, unpaid
invoices for years 2012, 2013 and 2014 are past due and payable. The total amount due is $2,580. I have
attached copies of the unpaid invoices for your review. Please see payment invoices for payment
instructions.
Failure to pay the fee in a timely manner in accordance with 15A NCAC 02H.0105(b)(2) may cause
this Division to initiate action to revoke the permit.
