13, 2023.

1. Please provide engineering certifications from a North Carolina licensed Professional Engineer for
the two septic tank effluent pump (STEP) units that service the Eco Lodge and Sea Base.
B. Application (Form: WWIS-R 02-21):
1. Pages 5 and 6 of Form WWIS-R 02-21 were not included in the renewal application package for
the Camp Boddie Wastewater Irrigation System. Please see Comments F-O below regarding the
missing information. Please note that a second Additional Information Request may be issued
depending on the information submitted on pages 5 and 6 of Form WWIS-R 02-21.
In the submitted application for Permit WQ0011655, Wayne Black is listed as the Back-up ORC
for the Camp Boddie Wastewater Irrigation System. However, Mr. Black has not been designated
as a Back -Up ORC for this facility. The Back -Up ORC currently designated for this facility is Louis
Delozier. To designate Mr. Black as the Back -Up ORC for this facility, please submit a Water
Pollution Control System Operator Designation Form (attached). It is important to note that the
Non -Discharge Branch does not process this form. Please submit the form to the Water Pollution
Control System Operators Certification Commission (WPCSOCC) by mail, fax, or email at the
