Staff Report
Application No.: WO0044990
Facility name: 312 Reese Rd SFR
Note: This form has been adapted from the non -discharge facility staff report to document the review of both non -discharge and NPDES permit applications and/or renewals. Please complete all sections m they are applicable.
I. GENERAL AND SITE VISIT INFORMATION
1. Was a site visit conducted? ❑ Yes or ® No
II. PROPOSED FACILITIES: NEW APPLICATIONS
1. Are the new treatment facilities adequate for the type of waste and disposal system? ® Yes or ❑ No
Site evaluation has not been conducted yet to determine if irrigation field is suitable, but other treatment
components are acceptable.
2. Are site conditions (soils, depth to water table, etc) consistent with the submitted reports? ❑ Yes ❑ No ® N/E
Site visit not conducted
3. Are the proposed application rates (e.g., hydraulic, nutrient) acceptable? ® Yes ❑ No ❑ N/A
If no, please explain: DC is 10%.
4. Are there any setback conflicts for proposed treatment, storage and disposal sites? ® Yes or ❑ No
If yes, attach a map showing conflict areas.
III. REGIONAL OFFICE RECOMMENDATIONS
1. Do you foresee any problems with issuance/renewal of this permit? ❑ Yes or ® No
2. List any items that you would like the NPDES Unit or Non -Discharge Unit Central Office to obtain through an
additional information request:
Item
Reason
Depth to bedrock
Confirm treatment and storage units will not be excavated into bedrock. Soil
profiles show that CR is as shallow as 24".
Soil profiles describe IOYR 6/1 mottles in Bt (4- —20") but no SHWT is
Depth to SHWT
described. VII.3 lists SHWT as 1.5ft. The soil boring log shows SHWT at 12-
14". Where is the SHWT?
• A stream is shown on the soil map but not on other maps. The soil map
shows a 50' setback from the stream, the required setback is 100'. I will
confirm the presence of a stream when a site visit is completed.
Setbacks
• Four additional houses are present within 250' of the proposed disposal
area, but only one setback waiver is provided. Houses are shown on
attached site map. Are additional houses 100' from proposed disposal
area?
The engineer calculations state the drip area is 5007 sqft (0.11 ac). VIIA lists
Field acreage
the drip field as 0.20 ac. The acreage is not included on the engineering plans.
What is the correct acreage of the drip field?
FORM: WQROSSR 04-14 Pagel of 2
The provided soil map does not include soil map units. PnE and PnD are both in
Soil Map
the immediate area. Slopes for PnE are 15-25% and PnD are 8-15%. A soil
map is needed to show the location of the drip field in relation to mapped
soil series.
• Listed the geologic unit as intermediate mafic unit, I found the system is
located in the Charlotte Belt unit.
• Described ksat horizon as "least expansive."
• Stated ksats were only measured in Bt horizon because lower horizons were
too thin or had poor structure. Poor structure can also limit movement of
Soil Report
effluent. Why wasn't a ksat completed in lower horizons? C horizon
was described as very firm, very sticky, and structureless. The C
horizon is >10" when described.
• Ksat Nest 2 had a significantly lower measurement than the other two nests.
The corresponding soil description describes a CR horizon starting at 19-
20". Is a discussion needed concerning the differences in the ksat
measurements?
The denial letter and SFR loading worksheet both use 480gpd. The design flow
480gpd
is 270 gpd. Is an updated SFR loading worksheet needed? Does the denial
letter apply to the proposed system as a 480gpd subsurface system was
denied?
Endangered Species
Are endangered species actually in the project area? The provided map shows a
larger project boundary.
3. Recommendation: ❑ Hold, pending receipt and review of additional information by regional office
