Division of Water Resources INSTRUCTIONS FOR FORM CSA 04-16& SUPPORTING DOCUMENTATION
Documents shall be prepared in accordance with 15A NCAC 02T .0100, 15A NCAC 02T .0400, and all relevant
Division Policies. Failure to submit all required items will necessitate additional processing and review time.

General — When submitting an application to the Municipal Permitting Unit, please use the following instructions as a
checklist in order to ensure all required items are submitted. Adherence to these instructions and checking the provided
boxes will help produce a quicker review time and reduce the amount of requested additional information.
The Applicant shall submit one original and one copy of the application and supporting documentation.
The copy may be submitted in digital format.
A. Cover Letter
® Submit a cover letter listing all items and attachments included in the permit application package
B. No Application Fee Required
➢ No application fee is necessary. The permittee will be billed an annual fee upon issuance of the permit
➢ The appropriate annual fee for systemwide wastewater collection system permits may be found at:

C. System-Wide Wastewater Collection System (FORM: CSA 04-16)Application:
® Submit the completed and appropriately executed System-wide Wastewater Collection System (FORM: CSA 04-
16) application. Any unauthorized content changes to this form shall result in the application package being
returned. If necessary for clarity or due to space restrictions, attachments to the application may be made, as long
as the attachments are numbered to correspond to the section and item to which they refer.
❑ If the Applicant Type in Section I.3 is a Privately-Owned Public Utility, provide the Certificate of Public
Convenience and Necessity(CPCN)from the North Carolina Utilities Commission demonstrating the Applicant is
authorized to hold the utility franchise for the area to be served by the wastewater collection system, or
❑ Provide a letter from the North Carolina Utilities Commission's Water and Sewer Division Public Staff stating an
application for a franchise has been received and that the service area is contiguous to an existing franchised area
or that franchise approval is expected.
❑ If the Applicant Type in Section 1.3 is a corporation or company, provide documentation if it is registered for
business with the North Carolina Secretary of State.

➢ The Authorized signing official listed in Section I.4 should match with that of the Applicant certification page in
accordance with 15A NCAC 02T .0106(b). Per 15A NCAC 02T .0106(c), an alternate person may be designated
as the signing official if a delegation letter is provided from a person who meets the criteria in 15A NCAC 02T
.0106(b).
➢ NOTE - Public Works Directors are not authorized to sign this permit application, according to the rule,
unless they are formally delegated.

Improvements
Outfall Rehabilitation—Phases 1 &2
During routine inspection, the existing 30-inch RCP sanitary sewer main that
stretches from the intersection of Boone Docks Street and Meadowview Drive to the
Jimmy Smith WWTP was found to be deteriorating. According to Town staff, an
effort (by Visu-Sewer of Ohio, LLC) to inspect the aforementioned sewer main with
a video camera and clean out the sewer main as needed (Phase
1) is scheduled to begin soon. The installation of cure-in-place pipe lining for
emergency repairs(Phase 2)is scheduled to begin following the completion of Phase
1.
A map of the proposed project is included in Figure 10. The total estimated cost for
the proposed project is approximately$276,000. It should be noted that Town staff
provided this unit price; however, a 15% contingency was added by McGill
Associates.

As previously mentioned, the existing 30-inch RCP sanitary sewer main that stretches
from the intersection of Boone Docks Street and Meadowview Drive to the Jimmy
Smith WWTP was found to be deteriorating. Following the completion of Phases 1 &
2, as previously described, cure-in- place pipe lining is planned to be installed in the
existing sanitary sewer main segments not rehabilitated in Phase 2.
For the purpose of this CIP update, it was assumed that 25% of the existing 30-inch
RCP sanitary sewer main would be lined through the emergency repairs
airs conducted in
Phase 2. Therefore, it was assumed that approximately 75% of the existing 30-inch
RCP sanitary sewer main (approximately 9,375 LF) would remain unlined after the
completion of Phase 2.
The proposed sewer project includes the installation of approximately 9,375 LE of 30-
inch cured- in-place pipe lining, bypass pumping, cleaning, and video inspections.
A map of the proposed project is included in Figure 10. The total estimated cost for
the proposed project is approximately $2,359,375.

Southgate Drive Bypass
Approximately 570 LF of existing 30-inch concrete sanitary sewer line is installed underneath
the Southgate Drive Strip Mall. The location of this sewer line makes routine inspection and
maintenance difficult.
The proposed sewer project includes the abandonment in place of the aforementioned
existing concrete sanitary sewer line,the installation of approximately 675 LF of 30-inch PVC
SDR 26 gravity sewer line, the installation of one 5-foot diameter standard precast concrete
manhole, and the modification of three existing manholes.
A map of the proposed project is included in Figure 11. The total estimated cost for the
proposed project is approximately $442,300.

New River Hills Lift Station
The Brookshire Road lift station (also referred to as the Industrial Park lift station) is
situated just northeast of the intersection of US HWY 421 and New River Hills. This lift
station collects sewer flow from the HWY 421 area northeast of the Town. An additional
lift station in this area would enable the existing Brookshire Road lift station to handle the
additional flow anticipated from the growing HWY 421 corridor.
The proposed sewer project includes the installation of a duplex submersible sanitary
sewer lift station with submersible pumps southeast of the intersection of US HWY 421
and New River Hills, the abandonment in place of approximately 87 LF of existing 12-inch
gravity sewer line, and the modification of three existing manholes. Additionally, the
proposed project includes the installation of approximately 100 LF of 12-inch PVC SDR 26
gravity sewer line and approximately 5,400 LF of 8-inch PVC SDR 26 sanitary sewer force
main.
A map of the proposed project is included in Figure 12. The total estimated cost for the
proposed project is approximately $1,828,300. It should be noted that the additional
expenses incurred due to land acquisition were not included in the aforementioned
estimate.

The Woodland Drive, Blanwood Drive, Oak Street, and Hillside Drive residential area is
currently served by 4-inch and 6-inch gravity sewer lines without manholes.These undersized
lines and the lack of sewer manholes violate 15A NCAC 02T regulations.
The proposed sewer project includes the replacement of the existing gravity sewer lines along
the aforementioned roads with approximately 4,400 LF of 8-inch PVC SDR 26 gravity sewer
line and twenty-four 4-foot diameter standard precast concrete manholes.
A map of the proposed project is included in Figure 13. The total estimated cost for the
proposed project is approximately$915,400.

Pump Station Contingency Plan
If a pump station failure is reported get a system operator to the scene as soon as possible,to assess
the pump station condition.Go through normal trouble shooting procedures and repair if possible.
If the pump station is in overflow condition refer to Utilities Contact list,contact Crew Supervisor
and or System Superintendent.Then proceed with contacting support help from approved pumping
contractor to set up and maintain bypass pumping until repair is complete. Then clean up any
debris and lime area.Estimate gallons ofspill and call NCDENR@ 336-776-9800 or(800-858-0368
after normal business hours)to report spill within 24 hours. Note the name of person answering
the report.
