On February 21, 2023, Melissa Joyner and Penny Markle from the Fayetteville Regional Office of the Division of Energy,
Mineral and Land Resources (DEMLR), conducted a site inspection for the Jordan Innovative Fabrication (formerly K-M
Machine Company) facility located at 275 Sedberry Road, Montgomery County, North Carolina. A copy of the Compliance
Inspection Report is enclosed for your review. Mr. Justin McBride, Human Resources and Safety Shop Manager was also
present during the inspection and his time and assistance is greatly appreciated.A Name/Ownership Change Form was sent
to this facility prior to the inspection but this form may not have been received and/or updates have not been made to our
database yet showing you as the owner and permit contact for this permit. The site visit and file review revealed that the
subject facility will be covered by NPDES Stormwater General Permit NCG030000 under Certificate of Coverage NCG030672.
Permit coverage authorizes the discharge of stormwaterfrom the facility to receiving waters designated as Hickory Branch,a
class C waters in the Yadkin River Basin.
As a result of the site inspection, the following permit condition violations are noted:
1) Stormwater Pollution Prevention Plan (SPPP)
Per Part B...A Stormwater Pollution Prevention Plan (SPPP) has not been properly developed and implemented.
2) Qualitative[ Monitoring
Per Part D, Section D-1...Qualitative Monitoring has not been conducted and/or recorded in accordance with permit
requirements.
3)Analytical Monitoring
Per Part E, Section E-1-E-4...Analytical Monitoring has not been conducted and/or recorded in accordance with permit
requirements.
Other Observations
Please refer to the enclosed Compliance Inspection Report for additional comments and observations made during the
inspection.
Reauested Response
You are asked to respond to this office, in writing, within 30 calendar days from receipt of this notice. Your written
response should include a reasonable explanation as to why the aforementioned violations have occurred as well as a
Plan of Action to prevent these violations from recurring.

Action Items

Inspection Summary:
Melissa Joyner and Penny Markle met with Justin McBride, Human Resources and Safety Manager at the Jordan Innovative
Fabrication (formerly K-M Machine Company, Inc.)facility. The Stormwater Pollution Prevention Plan (SPPP)was reviewed.
There had been a change in the ownership of this facility. Prior to the inspection, the Name/Ownership Change Form was
emailed to personnel at the facility.The form has not been submitted yet and our database has not been updated. Per
General Stormwater Permit NCG030000 requirements the following had not been implemented: 1.An annual feasibility study
about alternatives to current practices.; 2.A quarterly Stormwater Facility Inspection Program; 3. An annual documentation
of Employee Training; and 4. An annual evaluation of non-stormwater discharges from the outfalls. The Site Map should
include the location of the drains and note where the water is going to in these drains in the SPPP. The percent of
impervious area within each drainage area should also be denoted on the Site Map.Quarterly Analytical and Qualitative
Montoring has not been conducted.
During the review of the facility grounds an additional outfall (three total outfalls)was noted upslope of Outfall 2. The location
of this outfall will need to be shown on the Site Map.
