Division of Water Resources' Central and Regional staff have reviewed the application package
received February 14, 2023. However, additional information is required before the review may be
completed. Please address the items on the attached pages no later than the close of business on May 15,
2023.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Sections A through M, or failure to provide the additional information on or before the above requested
date may result in your application being returned as incomplete pursuant to 15A NCAC 02T .0107(e)(2).

1. Our records indicate that the Ocean Isle Beach WWTP had a major modification issued after
September 1, 2006. Pursuant to 15A NCAC 02T .0I05(d), please submit updated site maps meeting
the requirements in 15A NCAC 02U .0201(d). These requirements are: (1) A scaled map of the
site with topographic contour intervals not exceeding 10 feet or 25 percent of total site relief and
showing all facility -related structures and fences within 500 feet of the treatment, storage, and
utilization areas, (2) soil mapping units shown on all utilization sites, (3) delineation of the
compliance and review boundaries per 15A NCAC 02U .0501(a)(6), (4) setbacks as required by
15A NCAC 02U .0701, and (5) site property boundaries within 500 feet of all treatment, storage,
and utilization sites.

J. Compliance Schedules:
The existing permit includes a condition (Condition L1) regarding the conjunctive facilities
previously associated with rescinded permit (WQ0011614) for the Ocean Ridge Plantation WWTP.
This condition states that "prior to utilizing the reclaimed water storage and conjunctive irrigation
facilities at the Ocean Ridge Site, the Permittee shall obtain a permit, properly install and certify a
force main to convey reclaimed water from the Ocean Isle Beach WWTP to the conjunctive
reclaimed water storage and utilization sites at Ocean Ridge.
Please confirm that the Permittee has met the requirements of Condition L1 outlined in the permit
issuance dated August 29, 2018, and please provide the associated permit number for our records.
K. Civil Penalties and OUtstandinE Violations:

L. Setback Waivers:
1. The application states that the existing permit includes setback waivers; however, copies fo the
setback waivers were not included with the submittal documents. Please provide copies of the
setback waivers and include the accompanying Brunswick County Register of Deeds Book and
Page information.
M. Operation & Maintenance Plan:
1. A copy of the existing Operation & Maintenance (O&M) Plan was provided, but it does not appear
to meet the current requirements outlined in 15A NCAC 02U .0801. Please revise the O&M plan
to include the following:
i. A detailed description of the operation of the system to show what operations are necessary
for the system to function and by whom the operations are to be conducted;
ii. A sampling and monitoring plan designed to evaluate quality of reclaimed water within the
distribution system to provide quality assurance at the time of reuse, and including
specified actions to be take in response to unsatisfactory results;
In. A map of all reclaimed water distribution lines and record drawings of all reclaimed water
utilization systems under the permittee's control;
iv. A description of anticipated maintenance of the system;
v. A provision for safety measures, including restriction of access to the site and equipment
as required in Subchapter U; and
vi. Spill control provisions, including: (A) response to upsets and bypasses, including control,
containment, and remediation; and (B) contact information for plant personnel, emergency
responders, and regulatory agencies.
