the volume of wastewater to be permitted in this project has been evaluated along the route to the receiving
wastewater treatment facility and that the flow from this project is not anticipated to cause any capacity
related sanitary sewer overflows or overburden any downstream pump station en route to the receiving
treatment plant under normal circumstances, given the implementation of the planned improvements
identified in the planning assessment where applicable. This analysis has been performed in accordance
with local established policies and procedures using the best available data. This certification applies to
those items listed above in Sections I and 11 plus all attached planning assessment addendums for which I

The accompanied Fast-Track Application (FTA 06-21) includes 1,919 linear feet of 8" PVC public sewer and 4,277
linear feet of 12" PVC public sewer. The 12" line being installed is replacing existing 8" sewer within the Town of
Holly Springs's current sewer network that is undersized. The existing 8" line is being removed and replaced with
the 12" line listed in this permit. The sewer is planned to be removed and replaced in the same location, but new
pipes and manholes will be used for replacement of the upsized line. This work will occur roughly from the location
of Cayman Avenue/North Main Street all the way up to Sportsmanship Way/North Main Street in Holly Springs, NC.
The gravity sanitary sewer system for the proposed Main Street Vista development located at the corner of North
Main Street and Holly Springs Road in Holly Springs, NC drains to an existing public sanitary sewer main that is
located on the northwest portion of the site within North Main Street and is gravity fed north of the site. This sewer
line drains to an existing "Omni Pump Station"that the Town currently owns and operates. This existing pump
station is failing and is being proposed to be removed with this project. The pump station is planned to be removed
and a gravity system installed to take this offline. As a result,the entirety of this project and existing flows to this
existing pump station will now be directed to the existing "Twelve Oaks Pump Station" within the Town's system.
There are no upgrades planned with this project to this existing pump station because of these re-directed flows.
The sewer system will be a public system owned and operated by the Town of Holly Springs once it has been
accepted after construction. The project will consist of mixed-use buildings with a total of 277 residential units, an

The accompanied Fast-Track Application (FTA 06-21)includes 50 linear feet of 8"DIP public sewer and 1,850 linear feet of 8" PVC
public sewer. Also included is the removal and abandonment of the existing Omni(NC-55)aka Pump Station No.3 and associated 4"
sewer forcemain and appurtenances.Omni(NC-55)Pump Station is also known as"Pump Station No.3"in the 1985 Record Drawings
titled "Wastewater Collection and Treatment Facilities". Please see Exhibit"A"attached. This work will occur within the proposed
development, northeast of the corner of North Main Street and Holly Springs Road. The sewer from this development will tie-in to
existing sewer that runs north on North Main Street. This permit also includes work to install new gravity sewer from the intersection
of Quantum Drive and North Main Street to the east side of North Main Street adjacent to the existing business"Carroll's Tire and
Auto Center." The purpose of the sewer within the development is to serve the development itself. The purpose of the new gravity
line on North Main Street is to provide gravity sewer for the existing Omni Pump Station which is planned to be taken offline.The
removal of the pump station includes but is not limited to: removing chain link fence,electrical poles(coordinated with Duke Energy),
pump skid items,and electrical wires. Pumps,valves,and various other electrical equipment(e.g.—switch,antenna,etc)may be
salvaged, if possible,during demolition of the pump station.All conduits will be cut and capped off.The existing wet well will be
removed and backfilled.The existing force main leaving the pump station is planned to be abandoned in place.All activities associated
with the pump station abandonment will be confirmed and coordinated with the Town.
The entirety of this project will be directed to the existing"Twelve Oaks Pump Station"within the Town's system. There are no
upgrades planned with this project to this existing pump station because of these re-directed flows.
The sewer system will be a public system owned and operated by the Town of Holly Springs once it has been accepted after
construction.The project will consist of mixed-use buildings with a total of 277 residential units,an amenity,and commercial/retail
space.The total wastewater flow generated by this project is 73,079 GPD.The existing Omni(NC-55)aka Pump Station No.3 permitted
flows of 33,193 GPD will be diverted into the proposed system included with this permit application. The anticipated date for 1st
effluent discharge(certificate of occupancy)from the Main Street Vista project is September 2024.The 404 permit(SAW-2022-01030)
was submitted,and its approval was given on 9/27/2023. See the Table referenced below for permitted flow being diverted from
Omni Pump Station to Twelve Oaks Pump Station with this permit. This flow has been captured on the permits below and is tributary
to the plant;therefore, it is into included in this application.

I had a great conversation with Elaine from NCDEQtoday about the sewer permit review. She
recommended removal of the permit application specific to the Omni Pump Station Removal, since
the permit number is still unknown. She asked that instead, the Omni Pump Station Removal items
be included in the Main Street Vista permit app. The good news is that the fee for the Omni
application is no longer necessary. She will need the author of the check, or author's representative,
to either come by their offices to pick up the check, OR email her giving her permission for her to
destroy the check. She asked that this information be included in the email if that is the approach
you prefer.

Thank you for the information sent on 12/7/23. 1 appreciate your patience and assistance in
clarifying the scope of the project(s). To confirm, the RRO is reviewing two (2) application packages,
one new application for Main Street Vista Sewer Extension Application (includes replacing the Omni
PS and associated 4" FM with 8" GS) and one permit modification for NC-55 Roadway Improvements
Sewer Extension—WQ0003304 Modification. The permit applications are currently under review,
which will occur independently of each other, and any additional information requests sent as a
result of those reviews will need to be responded to separately.

In response to your initial email dated 10/24/23 and your subsequent email dated 10/30/23, please
find the attached three applications packages titled (1) Main Street Vista Sewer Extension
Application, (2) NC-55 Transportation Improvements Sewer Extension Modification WQ0003304M,
and (3) Omni (NC-55) aka Pump Station No. 3—Modification. Hard copies for all three packages and
checks for application packages (2) and (3) are being mailed via USPS today. Responses are shown in
blue italics below.
1 According to Section V1.7., the wastewater generated by the commercial and retail spaces
are 0.12 gal/day. Please provide detailed calculations specifying how this flow rate was
developed according to 15A NCAC 02T .0114.
Nick Williamson submitted the basis of calculations via email and signed/sealed pdf copy
on 1116123. A copy of those calculations is included in the resubmittal and via hard copy
of the Main St. Vista Sewer Extension Application
According to the narrative, approximately 4,277 LF of 8" gravity sewer is being replaced
with 4,277 LF of 12" gravity sewer described in this permit application. Please submit a
modification application to the Division to replace the existing line from its existing
permit. Additionally,this application will be placed on hold until the permit modification
for that gravity sewer is completed. Previously permitted infrastructure should not be
included in a new application for a fast track sewer extension. Can this permit
modification be submitted simultaneously with the permit modification required to take
the Omni pump station off-line? It appears that the project submitted should entail a
modification for the gravity sewer for NC55 Roadway Improvements, WQ0003304, and
the modification for the existing permit for the Omni Pump Station. If this is the case,
then the fast-track sewer extension application for Main Street Vista project application
should be withdrawn once the permit modifications for WQ0003304 and the Omni Pump
Station have been received. Both modifications can be completed concurrently with each
other.
o However, permit modification applications must be submitted with comprehensive
project narratives indicating what is covered in the existing permit and all information
related to the modification.
o If 2 modifications are replacing the Main Street Vista project, then we will transfer the
fee submitted with Main Street Vista to one (1) of the modifications so only one (1)
additional application fee will be required.
Main St. Vista site infrastructure and new flow will continue on a separate permit
application. An additional permit application titled "NC-55 Transportation Improvements
Sewer Extension Modification Application WQ0003304M"is included in the resubmittal
above and via hard copy for the 8-inch to 12-inch offsite sewer upsizing.
3. According to the narrative and Section VIII. of the application, the Omni Pump Station is
to be taken offline and replaced with the gravity sewer described in this application.
Please submit a modification application to the Division to remove the Omni Pump
Station from its existing permit. This application will be placed on hold until the permit
modification for that pump station is completed. Previously permitted infrastructure
should not be included in a new application for a fast-track sewer extension. Can this
permit modification be submitted simultaneously with the permit modification required
for the replacement of the 8" gravity sewer? It appears that the project submitted
should entail a modification of the existing permit for the Omni Pump Station and a
modification for the gravity sewer for NC55 Roadway Improvements, WQ0003304. If
this is the case, then the fast-track sewer extension application for Main Street Vista
project application should be withdrawn once the permit modifications for the Omni
Pump Station and WQ0003304 have been received. Both modifications can be
completed concurrently with each other.

3. According to Section I.h. of the FTSE form, 82%of the permitted flow will be used. Please
provide a Planning Assessment Addendum for Holly Springs' Utley Creek WWTP explaining
how the facility plans to increase capacity in order to meet the flow if all Paper Flows
become Tributary flow to the WWTP. The value in Section I.h. of 82% is based on the
combined Actual Flow received at the plant (section I.e) during the past calendar year+
the Obligated Flow (section I.d.). It is our understanding per 15A NCAC 02T.0118 (1) that
the addendum would be needed once the plant has exceeded 80%of its permitted
capacity based on the Actual Average Flow during the past calendar year. The UCWRF's
actual average flow in Section Le. in this application is 3.036 MGD which is 50.6% of the
6.0 mgd permitted plant capacity. Please advise.Thank you for the clarification. No
Planning Assessment Addendum required at this time.

4. According to Section V.1. of the application, Holly Springs Town-NC 55 Rdw (WQ0003304)
is the downstream receiving sewer. However, according to the FTSE form submitted by
the Town of Holly Springs, the downstream receiving sewer is the Twelve Oaks Regional
Pump Station and Force Main (WQ0030814). Please provide clarification on the
downstream receiving sewer. It was our understanding that for Section V.1 in the FTA, the
form is asking for the nearest sewer that the project discharges to. This project will tie to
an existing 12" sanitary sewer main that was permitted under WQ0003304. Whereas
Section II in the FTSE under pump stations, is asking for the downstream facility, which for
this project is the Twelve Oaks Pump Station that is permitted under WQ0030814. I hope
this helps. Please let us know if our interpretation is incorrect or you have additional
questions. Thank you for the clarification. No other information is needed at this time.

returned as incomplete. Any resubmittal of a returned application shall be accompanied
with a new application fee."
