As requested, below is the root cause analysis and countermeasures being taken to address the recent oil release by the
GFL compactor:
Root cause analysis:
Root causes:
• Ram Cylinder leak not detected resulting in release to storm drain.
• Trench storm drain located under the compactor is covered with a metal plate, but can allow oil to seep
into drain in the event of a leak or oil spills related to repair/replacement of cylinder.
• There was no containment in place or regular/formal inspections established in case of leak.
Countermeasures: (as agreed upon with Walter Skokow, Maintenance dept GFL);
• Install drip containment pan to catch oil in the event of leaks — Responsibility: Walter Skokow,
Maintenance, GFL estimated completion: Dec 2023
• Install Pig trench drain cover over drain directly under compactor— Responsibility: Mike Brown, Electrolux,
estimated completion: Dec 2023.
• Establish weekly inspection/PM of compactor condition/leaks. -Responsibility : Walter Skokow, Maintenance,
GFL estimated completion Dec 2023.
• Add to SWPPP weekly inspection for evidence of oil leaks at compactor. Notify GFL if oil leaks
detected. Resp. Mike Brown, Electrolux. Est. completion Dec 2023.
1 will follow up with objective evidence of completion once items are completed. Please let me know if you have any

I checked the values I had entered in edmr. The "<"values are entered correctly.

Evidently, the summary you printed out is not picking up the "<" value. Unless otherwise directed, I will take no further
action on this issue.
Regarding GFL compactor containment improvement, I will update you as once this is in place.

My colleague, Bill and I are planning an inspection of Electrolux facility in Kinston, NC to review the
following:
1. Compliance NPDES stormwater inspecton (NCG030450), and
2. Any other stormwater pollution/exposure related issues at this facility.
This inspection is scheduled for Wednesday, Nov 29, starting around 10:30 am. In the meantime, please
send the annual summary report for last 2 years.
