Inspection Summary:
A routine compliance evaluation was conducted on August 9, 2023, by Morella Sanchez King with the Wilmington Regional
Office, DWR Water Quality Regional Operations Section. The Division appreciates the assistance provided by Mr. Dan
Fortin, Robert Howard, Kevin Stanley, and David Kane.
The purpose of the evaluation was to verify that the facility is operating in compliance with the requirements of permit
WQ0000224.
Permit: A new permit was issued on January 20, 2022. Please read the permit carefully since some conditions may have
been added or modified.
Permit component description is in accordance with components on site.
Permitted flow: 52,950 gpd
Average flow: —5,000 gpd
Response to previous inspection requests: The 2021 inspection reported indicated that the generator on site had been
inoperable for 3-4 months. A new generator was ordered and has been installed and in operation. The generator was
exercised during inspection and performed satisfactorily.
Performance Standards:
1) Groundwater monitoring reports (GW59): There have been recurrent exceedances of TOC in monitoring well #4
(November 2021, March 2022, July 2022, March 2023). MW-4 is distant from the effluent application sites (rotary distributor
field and low-pressure subsurface system) and in proximity to a roadside ditch that runs parallel to Coast Guard Dr. This
location, near the road, might be the cause of the recurrent TOC exceedances. However, MW-4 can serve as a background
well given its distance to the disposal sites, away from the influence of the WWTP operation.
2) Non -Discharge monitoring reports (NDMR): The facility has not been issued notices (NOD or NOV's) since 2017. There
was a fecal coliform daily max exceedance in June 22 with no violation of the parameter for the month. No notice was
issued.
3) Operation of the Facility: DWR commends the work of the operators to keep the effluent limits in compliance with
Attachment A requirements (see item 2, above).
4) Maintenance: Several areas of the facility need repairs because of the advanced state of deterioration and corrosion
which in some areas represents a safety hazard for operators, inspectors, and any visitors. This is a violation of the permit
PERFORMANCE STANDARD (11.1) whose description prevents facility failure from lack of maintenance. The facility is
required to have an Operation and Maintenance Plan indicating the maintenance schedule of the entire structure of the
facility (OPERATION AND MAINTENANCE REQUIREMENTS (111.1) The enclosed photos evidence that the structure of the
facility has not received adequate maintenance.
5) It is recommended to install a telemetry system for the influent pump station. The pump station has audio and visual
alarms which are audible/visible if operator(s) are at the plant, or maintenance personnel or someone using the trash
dumpsters are nearby. Otherwise, operators rely on someone to call them if the alarm is heard. A telemetry system is a
requirement for pump stations designed for two pumps, this is described in Pump Station Reliability [15A NCAC 02T
.0305(h)(1)(E)].
A Notice of Violation will be issued for the state of deterioration and corrosion which in some areas represents a safety
hazard for operators, inspectors, and any visitors.

include the followinq components: a manual bar screen, dual aeration tanks (36,000 and
18,000 gallon -capacity), dual blowers, dual clarifiers, and dual sand filters. The treatment and
disposal components at the site are consistent with their descriptions in the 2022 permit.

Comment: The influent pump station (PS) has audio and visual alarms which are audible o visible if
someone is at the WWTP or near the PS (i.e. maintenance personnel or someone using the
garbage dumpsters would call the WWTP operator). Otherwise, the audio/visual alarm could
be overlooked. A telemetry system is required for all PS designed for two pumps or more.
This is to provide remote notification of a problem condition, including power failure and high
water alarm fRef: 15A NCAC 02T .0305 (h)(1)(E))1.

Comment: Dissolved oxygen (DO) is in the range of 1.2-1.5 mg/I. Air seemed to be evenly distributed
through the aeration basins. Color was adequate.

Comment: The effluent disposal system consists of (1) a rotary ditributor and (2)
a historically permitted
low-pressure pipe subsurface disposal system. Effluent is alternatively
disposed off in both
systems (50% and 50%). On inspection day, one of the axis of the rotary distributor had some
nozzles clogged or not working properly.
