Enclosed you will find the report entitled Eleventh Supplement, Phase 2 Ground Water
Quality Assessment, prepared by our consultant, Nutter & Associates. We submit this report as
partial fulfillment of the requirements in the October 18, 1996 Notice of Violation associated with
the original Permit No. WQ0002377. These fields (fields 26-70) have been transferred to Permit
No. WQ0008764 and renamed NC-SC-3-1. The original permit WQ0002377 has been rescinded.
The geometry of the nitrate -nitrogen plume around the old grassed fields (fields 26 - 70) is
similar to the configuration reported following the March 2003 monitoring event; however, the
concentrations are substantially lower than in March 2003 or March 2004. The March 2005 data
indicate a continuation of the sharp decrease in ground water nitrate -nitrogen beneath fields 26 -
70.
At temporary wells, B, D, E, and F, ground water remains below the 10 mg/L MCL for nitrate -
nitrogen. Ground water nitrate -nitrogen at LAM-3 has also decreased below 10 mg1L.
Concentrations at wells C and LAM-2 at the south property boundary remain above the MCL but
have decreased compared to past high levels, as have concentrations at wells G2 and LAM 1-
COMP. The decreasing trend in nitrate -nitrogen concentrations around these fields is in response
to the termination of biosolids_application to the�e�elds —.�.__ "-' -

c. Each land application site identified with a GW-E in the most recently -certified
Attachment B of this permit may include artificial subsurface drainage systems at greater
than four feet of depth.' A surface water monitoring plardstudy has been agreed to
between the Division and the Permittee, as a reult of a September 27, 1994 meeting. The
plan shall be followed and the surface waters monitored to ensure the land application of
the subject residuals does not violate State laws, rules and regulations. Any drainage
system that is less than 3' feet from the ground surface 'shall have the proper buffer
established.
2. Applicable Boundary Requirements:
a. The COMPLIANCE BOUNDARY for residuals land application programs is specified
by regulations in 15A NCAC 2L (i.e., "Groundwater Classifications and Standards").
The Compliance Boundary,for each land application site is established at either 25.0 feet
from the residuals land application area or 50 feet within, the property boundary,
whichever is closest to the residuals land application area. An exceedance of
Groundwater Quality Standards at or beyond the Compliance Boundary is subject to
immediate remediation action according to 15A NCAC 2L .0106. (d)(2).
b. The REVIEW BOUNDARY shall be established around each land application site
midway between the Compliance Boundary and the perimeter of the residuals land
application area. Any exceedance of Groundwater Quality Standards at the Review
Boundary. shall require action in accordance with 15A NCAC_2L .0106 (d)(2).
3. Sampling Criteria:
a. Monitor wells 14VI, and 1440n old Site 1 o field 1 wC- •- -0,1'
and monitor wells MW- , MW-10, MW-11, MV�12, and 1 o d Sit (old
field 3 which is now divided to VIA'SC'U3T4 and NC-S03I3 shall be
sampled every March, July and November for the'parameters listed below. Monitor
wells MW-4, MW,-5, MW-6, MW-7 and MW4 on Site 2. (old_ field 2 which is no
longer in the land application program). shall be sampled every November for the
parameters listed below. Prior to sampling the parameters, the measurement of water
levels must be taken. The depth to water in each well shall be measured from the
surveyed point on the top of the casing. The measuring points (top of well casing) of
all monitoring wells shall be surveyed relative to a common datum.
Water Level pH
Chloride Nitrate Nitrogen (NO3-N)
Total Organic Carbon Total Ammonia Nitrogen (NH3-N)
Total Dissolved Solids Fecal Coliform
b. Monitor wells MW4, MW-5, MW-6, MW-7 and MW-8 on Site 2 (old field 2 which is
no longer in the land application program) shall be sampled every November for the
parameters listed below. Prior to sampling, the parameters, the measurement of water
levels must be taken. The depth to water in each well shall be.measured from the
surveyed point on the top of the casing. The measuring points (top of well casing) of
all monitoring wells shall be surveyed relative to a common datum.

Chloride Nitrate Nitrogen (NO3-N) '
Total Organic Carbon Total Dissolved Solids
Mercury Volatile Organic Compounds
C. For Total Organic Carbon (TOC), if concentrations greater than 30 mg/I are detected in
any down -gradient monitoring well, additional sampling and analysis must be
conducted to identify the individual constituents comprising this TOC concentration. ,If
the TOC concentration as measured in the background monitorwell exceeds.10 mg/l,
this concentration will be taken to represent the naturally occurring TOC concentration.
Any exceedances of this naturally occurring TOC concentration in the down -gradient
wells shall be subject to the additional sampling and analysis as described above.
d. For Volatile Organic Compounds (VOCs) use only one of the following methods:
1) Standard method 623D, Practical Quantitation Limit (PQL).at 0.5 ug/L or
less
2) Standard Method 6210, PQL at 0.5 ug/L or less
3) EPA Method 8021, Low concentration, PQL at 0.5 ug/L or less
4) EPA Method 8260, Low concentration, PQL at 0.5 ug/L or less
5) Another method with prior approval by the Groundwater Section Chief.
C. Any laboratory selected to analyze parameters must be Division of Water Quality
(DWQ) certified for those parameters required.
4. Reuortim / Documentation:
a. All reports and documentation (GW-59) shall be mailed to the following address:

Updated blank. forms (GW-59) may be downloaded from the Aquifer Protection
Section's website at http://gw.ehnr.state.nc.us/ or requested from the address mentioned
above.
b.The results of the sampling and analysis must be received on Form GW-59
(Groundwater. Quality Monitoring: Compliance Report Form) by the Aquifer
Protection Section (address listed in the Reporting / Documentation: section of the
Groundwater Requirements), on or before the last working day of the month following
the sampling month. The data of all groundwater sampling analyses required by the
permit conditions must be reported using the most recent GW-59 form along with
attached copies of the laboratory analyses.
c. Within sixty (60) days 'of permit issuance, the permittee shall submit two original
copies of a scaled' site map (scale no greater than 1"=100') for Site NC-SC-02-14;
however, special provisions may be granted upon prior approval for large properties.
The map shall show all of the property boundaries within 500 feet of the disposal area,
and the location of Review and Compliance boundaries.
The permittee is responsible for the geographic accuracy of any map submitted,
however produced. ,

5. Additional Requirements:
a. The three sprayfield areas designated as NC-SC-03-01, NCSC-03. 03A and NC-SC-03-
03B shall not receive residual applications until such time as the concentrations of all
parameters in all eight monitor wells (MW-l.through MW-3 and MW-9 through MW-
13) are detected at concentrations which met the Groundwater Quality Standards
specified under 15A NCAC 2C .0200. When this takes place, the applicant shall
contact the Aquifer Protection Supervisor of the Fayetteville Regional Office prior to
the application of residuals and a determination shall be made as to whether the
application of residuals on these fields can take place. The Groundwater Section shall
also determine at this time whether a continuation of groundwater monitoring is
warranted.
b. Any groundwater quality monitoring, as deemed necessary by the Division, shall be
provided.
V. INSPECTIONS
1. Prior to each residuals land application event, the Permittee or his designee shall inspect the
residuals storage, transport; and application facilities to prevent malfunctions and
deterioration, operator errors, and discharges that may cause or lead to the release of wastes
to the environment, a threat to human health, or a nuisance. The Permittee shall maintain an
inspection log or summary including at least the date and time of inspection, observations
made; and any maintenance, repairs, or corrective actions taken by the Permittee. This log of
inspections shall be maintained by the Permittee for a period of five years from the date of the
inspection and shall be made available to the Division or other permitting authority, upon
request.
2. Any duly authorized officer, employee,. or representative of the. Division may, upon
presentation of credentials, enter and inspect any property, premises, or place on or related to
the land application sites or facilities at any reasonable time for the purpose of determining
compliance with this permit; may inspect or copy any records that must be kept under the
terms and conditions of this permit; and may obtain samples of groundwater, surface water,
orleachate.
VI. GENERAL CONDITIONS
1. This permit shall become voidable unless the residuals land application events are carried out
in accordance with the conditions of this permit, the supporting materials, and in the manner
approved by the Division.
2. This permit shall be effective only with respect to the nature and volume of residuals
described in the application and other supporting data.
3. Failure to abide by the conditions and limitations contained in this permit may subject the
Permittee to an enforcement action by the Division in accordance with North Carolina
General Statutes §143-215.6A through §143-215.6C.
