QUALIFICATIONS AND TRAINING OF OWNERS AND OPERATORS The operator of the facility is James & James Environmental Mgt.,lnc.. The breakdown is as follows: Juanita James -worked in environmental field since 1991; has operated numerous systems consisting ofwastewater, water, subsurface,spray irrigation, collections,distribution, community, non -community, and transient systems. She holds certificates in all of these fields. One or more of the following employees have been assigned to your facility. B. Certificates are received and submitted with each license renewal at the end of each calander year. This training is an approved course through the Public Water Supply. 3) POLICIES A. RESPONSE AND NOTIFICATION IN CASE OF VIOLATIONS All violations received by the owner are forwarded to the office of James & James Env. And is researched within our files. A phone call or e-mail correspondence is then made with the governing body to either make arrangements to re -send the needed paperwork. The office of James & James Env follows up with the particular engineer to confirm that the violation has been satisifled. B. SAFETY PROCEDURES James &James Environmental is very safety oriented. Safety glasses are to be worn to prevent accidental splashing into the eyes and wipes are provided to clean up any exposed skin. We require our operators to be around the metal piping the most minimum time during thunder storms to lessen the possibility to be struck by lightning. All service trucks are 4x4's to lessen the possibility of accidents during inclement weather. C. PROCEDURE FOR KEEPING AND COMPILING RECORDS An individual file is maintained at James &James Env.. There are no files discarded, even if the allotted time has passed. lames & James Env works with the systems to ensure that all correspondence received by the system is forwarded to James & James Env. 4) EMERGENCY MANAGEMENT PLAN: A. CALL911 B. LOCK CONTROL PANELS AND LIFT STATION LIDS C. IDENTIFY POTENTIAL SOURCES OF CONTAMINATION D. IDENTIFY LOCAL POLICE, PUBLIC HEALTH OFFICIALS; PRACTICE AN Page 1 of 3 F. EMERGENCY/SPILL RESPONSE PLAN 1 Ilan environmental hazard, call 911, then 2 contectJuani,a James, and 3 Juanita James will contact DWQ in Swannanoa, and 4 Juanita James will contact the owner or designated representative of the system, and �5 Juanita James, or maintenance personnel will arrive on site quickly and stay for the duration of the repair. Any and all persons and items needed for the emergency will he deployed right away and will be on site until the emergency is corrected. ©James and James will complete any documentation for DWQ as well as the 5-day follow-up response 5) OPERATION AND MAINTENANCE PLAN: A. CAPACITY OF PUMPS AND STORAGE TANKS B. DOCUMENT NORMAL OPERATIONS The assigned ORC or its back-up will arrive on site either daily or weekly depending on telemetry to check system. C. SOP'S 1. PLANT START-UP, SHUT DOWN, NORMAL OPERATION PlantStart-up should only occur after a power outage. Ensure first that the breakers have not been tripped and that all equipment is in operation mode. Second, start the equipment and watch and listen for problems. Call with problems. Plant shut down should only occur when repair is being made to the system and should be done by breaker. 2. RECORD KEEPING A log book is kept on site, documendngall tests and workperformed. - 3. DECISIONS REGARDING PUBLIC HEALTH Decisions regarding public health are made by joint effort including the Owners of James & James Environmental, the system representative and the Regional Office. If notification is needed, the recipients are notified by either phone, letter or posting in a public area. 4. MAINTAIN RECORDS INCLUDING EQUIPMENT REPAIR (COST AND REASON) All equipment that is replaced is first approved by the system representative and is recorded on the invoice to the system provided by James& James Environmental. A work order listing the item, cost and installation is maintained by James &James Environmental Mgt., Inc. S. MAINTAIN RECORDS IN A EASILY ACCESSIBLE LOCATION All records are kept at the offices of)ames & James Environmental Mgt, Inc. and are available for review at any time by the State representative or system representatives. Any request of records are first approved by the system representative. 6. KEEP CONTACT INFORMATION ACCESSIBLE Maintained by the James & James Environmental Mgt, Inc. office personnel and is carried with Juanita James for quick response. Page 3 of 3 E.
