Please see the attached forms as requested. We are well below the 300 feet for permanent impacts on perennial
streams; Most impacts are temporary.
I'll do a better job in the future coordinating with the engineer for their labelling and our PCN/impact maps - hopefully
this will help streamline things.

The issued PJD is helpful for the wetlands located on -site! Since the PJD only states "non -wetland waters" for the
tributaries, DWR will request stream identification form(s) when mitigation is required because we need to be able to
distinguish between the two for our records and for mitigation purposes. Since we require mitigation on perennial
impacts over 300 LF, we typically request those stream form(s) to have in our records for future use/reference, etc. On
top of this reason, I'm asking for the stream form(s) because when you initially submitted your PCN, your maps
indicated all streams located on -site are perennial, but in your PCN, S1, S2, S3, and S7 indicate intermittent stream
impacts. Since there are discrepancies with your PCN/maps, I'm requesting the stream forms for clarity. [15A NCAC 02H

The applicant is not requesting concurrence from DWR on streamflow regime, and I am not familiar with the
requirement to have a DWR stream ID form for each feature. As DWR jurisdiction extends to all US Army Corps
regulated features, the issued PJD has been provided to document the presence of jurisdictional streams/tributaries on
the site.

1. Please provide an additional overview map with clear parcel boundaries of the project area.
The current maps (Figure 1, Stream Crossing — Existing Conditions, and Stream Crossing —
Overall Site Plan) are hard to understand. See attached SheetX-114 Full Overall Site Plan
2. On Figure 1 supplied by Wetlands & Waters, Inc. there are four tax pins provided
(3543075624, 3533779760, 3533789387, 3533795446). There are only three listed on the
map. Are there four parcels included in this project area? If so, please provide the deed
information. If the fourth parcel is being included, can that area be used to minimize
impacts?
This parcel is included (Deed Book 4104/Deed Page 1817). It is composed entirely of uplands
and was added to the assemblage and utilized to maximize avoidance
and minimization by moving the road crossing above the existing stream and wetlands.

3. During review of the impacts, there are unaccounted impacts listed in the PCN table and on
the figures. Please update the PCN table/map(s) accordingly. Please provide a construction
drawing / plan sheet that labels and includes all permanent and temporary stream and
wetland impacts to clearly document impacted wetland limits and wetland areas which are
proposed to be avoided the proposed construction and development activities. Please
provide the impacts in these units: Wetlands Impacts = Acres and Stream Impacts = Linear Feet.
See attached updated drawings with impact numbers corresponding to impact tables for clarity.
