Director NP Loden Durham, LLC Amy Rogers c/o York Properties Inc 2108 Clark Avenue Raliegh, NC 27605 NORTH CAROLINA Environmental Quality December 13, 2023 Re: Issuance of Injection Well Permit Permit No. WI0500180 Geothermal Heating/Cooling Water Return Well Durham County Dear Amy Rogers, In accordance with your permit renewal application received July 25, 2023, I am forwarding Permit No. WI0500180 for the continued operation of geothermal heating/cooling water return well(s) located at the above referenced address. Please note that this renewed permit shall become effective date of issuance until November 30, 2028, and shall be subject to the conditions and limitations stated therein. Please note: • Within 90 days of permit issuance, contact the Raliegh Regional Office (RRO) and verify correct nomenclature of the wells and their locations. The attached map identifies the current understanding of the location and names of these wells. • Within 90 days of permit issuance, clearly identify and label all the wells and their sample ports, and provide the DWR with proof (photograph of the wells and sample ports, or additional inspection) that this has been done. • Within 90 days of permit issuance, have a licensed well driller inspect the vault and well seal for W2, make repairs as necessary, and provide a written verification to the RRO summarizing these repairs and affirming that the vault and well seal are sound. The DWR strongly recommends either removal of the planter from this area, or reconstruction of the vault to prevent water from entering into the vault. • Within 90 days of permit issuance, ensure that the vault for well WI is readily accessible. • The laboratory analytical results for the sampling of the wells is pending, and the RRO will provide these to you when they are available, and will identify if there are any results that might impact the continued operation of these wells. In order to continue uninterrupted legal use of this well for the stated purpose, you should submit an application to renew the permit 120 days prior to its expiration date. As indicated in the permit, this permit is not transferable to any person without prior notice to, and approval by, the Director of the Division of Water Resources.
