Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application: Existing conditions include SR 1654 (Morton Road), agricultural land, undeveloped forest, and disturbed/maintained right-of-way. 4b. Explain the purpose of the proposed project: * To remove Bridge #030217 and replace with a new, modern bridge structure. The project was formerly designated as B-5795 due to federal funding that was previously proposed and was also formerly known as 17BP.10.R.140 when state funding was proposed. 4i. Describe the overall project in detail, including indirect impacts and the type of equipment to be used: * Erosion and sedimentation control measures will be installed including impervious dikes around the bridge piers for dewatering. Bridge demolition will include the removal of the bridge deck, the piers and bridge abutments, and footings. Estimated temporary impacts to Lanes Creek are 108 linear feet (LF), including 55 LF for dewatering, associated with demolition and construction activities. Additional potential temporary impacts to the intermittent unnamed tributary (UT) to Lanes Creek of 74 LF are also anticipated. All totaled, anticipated temporary impacts to streams are 182 LF. Permanent impacts of 12 LF to the UT to Lanes Creek will result from the riprap ouffall pad and an additional 16 LF of permanent stream impact will result from excavation to the UT required for abutment protection; all totaled, permanent stream impacts encompass 28 LF. The existing bridge is a 135.3' (1@45'2", 2@45'1") triple -span bridge with a steel plank deck on I -beams and mass concrete abutments and interior bents. The proposed structure will be a 170' long bridge with a span arrangement of 1@50' (21" cored slab), 1 @70' (24" cored slab) and 1 @50' (21" cored slab). The proposed roadway alignment over the bridge is the same as the existing bridge alignment. Overall drainage patterns will be maintained. There are roadside ditches that flow towards the bridge on either side of the structure. The drainage pattern of the ditches is being maintained and the drainage from the bridge is being discharged into the ditches. Deck drains will be needed for the proposed structure to prevent spreading inside the travel lane. The deck drains will not discharge directly to the jurisdictional stream. Deck drains will be located over the floodplain bench area as shown in the BSR. Impervious dikes with special stilling basins shall be used to contain the sediment laden water and prevent it from being discharged into the stream. The proposed bridge will be built on the same alignment with a temporary off -site detour, that includes Lee Road, Rocky River Church Road, Morton Road, and Hough Road. Heavy construction equipment and cranes will be used for the bridge demolition and construction with the work being conducted from upland roadway approaches. 5. Jurisdictional Determinations 5a. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques: NCDOT Best Management Practices for Construction and Maintenance Activities will be employed. Demolition and construction methods from the upland roadway approaches will be used to minimize potential temporary demolition related impacts and avoid construction related impacts. 2.
