Dear Mr. Lowery,
On June 13, 2023, Curtis Tyree from the Raleigh Regional Office visited your single-family
residence (SFR) wastewater treatment system to evaluate compliance with the subject General
NPDES Permit.
Our records indicate the treatment system consists of a septic tank; a below ground primary sand
filter; a chlorinator; a discharge pipe; and a rip -rap apron for post aeration.
General NPDES Permit NCG550000 and Certificate of Coverage (COC) NCG551119 authorize
the discharge of domestic wastewater from your treatment system to Hyco Lake.
Findings during the inspection were as follows:
1. The septic tank shall be checked annually and pumped out every 3 to S years. Mr. Lowery
stated that the septic tank had been pumped out in 2017 and he has had a contractor to
inspect the system in 2021 and the tank didn't need to be pumped out at that time. He will
have the contractor to come out and inspect the system again this year and email the results
to Curtis Tyree.
2. Treatment system operation. The treatment system shall be maintained at all times to
prevent seepage of sewage to the surface of the ground. Mr. Lowery inspects his system on
a regular basis.
3. Chlorination. The tablet chlorinator shall be inspected weekly to ensure there is an
adequate supply of tablets for continuous and proper operation. Wastewater grade tablets
(calcium hypochlorite) shall be added as needed to provide proper chlorination
(swimming pool chlorine tablets shall not be used). At the time of the inspection, the

chlorinator had a sufficient amount and type of tablets and Mr. Lowery inspects the tubes
often.
4. Outfall location. A visual review of the outfall location shall be executed twice each year
(one at the time of sampling) to ensure that no visible solids or other obvious evidence of
system malfunctioning is observed. Any visible signs of a malfunctioning system shall be
documented, and steps taken to correct the problem. At the time of the inspection, the
outfall location was clear and appeared to be well maintained and free of any obstructions.

5. Effluent sampling requirements. Effluent sampling must be conducted annually as part of
your permit requirements. The effluent samples must be analyzed by a North Caroline
Certified Lab and the results must be kept on site for three years. Mr. Lowery has not been
able to collect a sample yet this year due to no flow to the out fall but will collect one as
soon as he detects a flow.
6. Fees and renewals. COC's with unpaid administering and compliance monitoring fees
will not be automatically renewed The fees must be paid annually and within 30 days of
notification. All fees have been paid.

Septic tank was pumped out in 2017. The permittee is the only one staying in the home. An
inspection in 2021 noted that the system didn't need to be pumped at that time. The permittee will
have someone to inspect the system again this year.
The sand filter appears to be assimilating the waste as designed.
The chlorinator had the correct type and amount of tablets.
The outfall was located and accessable.
