Chapel Hill, NC 27516 NORTH CAROLINA Envirorunenral Quafity July 12, 2023 Subject: Compliance Evaluation Inspection Single Family Wastewater Treatment System General NPDES Permit NCG550000 Certificate of coverage NCG551566 Facility: 5722 Old Greensboro Hwy Orange County Dear Mr. Blake, On June 2, 2023, Curtis Tyree from the Raleigh Regional Office visited your single-family residence (SFR) wastewater treatment system to evaluate compliance with the subject General NPDES Permit. Our records indicate the treatment system consists of two septic tanks; a recirculation tank; two AdvanTex pods; a control panel with telemetry and Ultraviolet disinfection; and an out fall discharge pipe. General NPDES Permit NCG550000 and Certificate of Coverage (COC) NCG551566 authorize the discharge of domestic wastewater from your treatment system to an unnamed tributary to Persimmon Nursery Branch to coIlins Creek, classified WS-IV NSW waters in the Cape Fear River Basin. Findings during the inspection were as follows: 1. The septic tank shall be checked annually and pumped out every 3 to S years. The system is under contract by AWT and is inspected two times per year. As of the last time the system was inspected, the septic tank did not need to be pumped. 2. UV disinfection. The UV disinfection was working at the time of the inspection. The alarms were set up on telemetry and AWT inspects the system two times per year. 3. Treatment system operation. The treatment system shall he maintained at all times to prevent seepage of sewage to the surface of the ground. At the time of the inspection, the system appeared to be well maintained and AWT inspects the system two times per year. A visual review of the outfall location shall be executed twice each year (one at the time of sampling) to ensure that no visible solids or other obvious evidence of system malfunctioning is observed. Any visible signs of a malfunctioning system shall be documented, and steps taken to correct the problem. At the time of the inspection, the outfall location was clear and appeared to be well maintained and free of any obstructions. There was no water discharging at the time of the inspection. 5. Effluent sampling requirements. Effluent sampling must be conducted annually as part of your permit requirements. The effluent samples must be analyzed by a North Caroline Certified Lab and the results must be kept on site for three years. The last effluent sampling was performed in November 2022 and the results were within the permit limits. Sampling in 2023 has not been conducted at the time of this inspection. 6. Fees and renewals. COC's with unpaid administering and compliance monitoring fees will not be automatically renewed. The fees must be paid annually and within 30 days of notification. All fees have been paid.
