M4 The spike recovery value was unusable since the analyte concentration in the sample was disproportionate to
the spike level. The associated Laboratory Control Spike recovery was acceptable.

This sample was stored in a refrigerator that malfunctioned causing the sample to freeze. The state was notified and
the results may be reported for compliance purposes as long as the data is flagged. Any parameter requiring
preservation at < 6C and that was tested after 3/25/2015 at 15:45 when the refrigerator was documented to be in
compliance are flagged with N1.
