Remarks and Recommendations:
The "ICA" status on this project has been lifted. A follow-up review was conducted on Sept. 8, 2015 with Chris
Kirkman, Chris Fine, Jody Andrews, w/DOT, and contractor personnel that indicated corrective actions have
been taken to bring the project back into compliance.
Please note the following comments and recommendations:
The contractor has installed erosion control measures, as per plans.
TRSC's/SSCF have been installed in four corners at wingwalls of culvert and silt fence around the headwall
area.
Off-site sediment in stream has been removed by hand, as per Jerry Parker, DEO/agencies.
Approx. sta.12+50 Lt.- need to stabilize small area of slope above rock, as discussed.
General comments: Contractor has stabilized roadbed at this time. Once shoulders/ditches are final graded,
ensure e. c. measures are re-installed and disturbed areas are permanent seeded/matted. Please be reminded
that the contractor is responsible for taking all reasonable measures to prevent off-site sediment. This includes
surveying the project at end of each workday to ensure e. c. measures are installed, as per plans and that
additional measures are installed, as needed.
