4. Project Description and History
4a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application: *
3597 Browns Pond Rd, Selma, NC 27576 is the site of my (Logan Williams) personal home, and 13 acre property. The specific site in question is a small unnamed creek with a riparian
buffer in a natural/wooded area. The majority of our 13 acre property is on the north side of this small unnamed creek. My family cannot access the overwhelming majority of our property,
due to our home being on the other side (south side) of the small unnamed creek. This small unnamed creek does not have a name, and appears to only be about 2 inches deep in water
normally.
4b. Have Corps permits or DWR certifications been obtained for this project (including all prior phases) in the past? *
Yes No Unknown
4b(2). Is Any Portion of the Work Already Complete?
Yes No
If yes, describe the completed work.
4c. List of Other Certificates or Approvals/Denials received from other Federal, State, or Local Agencies for Work Described in This Application. not related to the 404 or 401.
None
4f. List the total estimated acreage of all existing wetlands on the property:
1 acre
4g. List the total estimated linear feet of all existing streams on the property:
1,OOOfeet
4h. Explain the purpose of the proposed project:
My family and I only need a permit for a small bridge we can build over the small unnamed creek. This will then allow us to enjoy the majority of our property at home. We could then do
activities including hiking, hunting, and enjoying the nature on our land.
41. Describe the overall project in detail, including indirect impacts and the type of equipment to be used:
The proposed bridge would run perpendicular across the creek, to minimize its size. This bridge would also only be wide enough for one normal sized vehicle to cross at a time. The bridge
would use a large culvert(s) to ensure the water is not hindered. The amount of surface water affected by this bridge would be far below 0.05 acres. I am willing to adjust these plans to be
within regulations, guidelines, and to have as little effect on the creek as possible.
4k. Will this activity involve dredging in wetlands or waters?
Yes No
If yes, Describe the type of dredging, the composition of the dredged material and the location of disposal area
5. Jurisdictional Determinations
5a. Have the wetlands or streams been delineated on the property or proposed impact areas?*
Yes No O Unknown

6. Future Project Plans

E. Impact Justification and Mitigation
1. Avoidance and Minimization
la. Specifically describe measures taken to avoid or minimize the proposed impacts in designing the project:
This creek runs completely across property. This causes there to be no access to the majority of our 13 acre property without this bridge. Only one single
lane bridge will be constructed to minimize impact.
O
1b. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques:
Bridge crossing will be 90 degrees perpendicular to creek, to minimize impact. Most work will be done by hand, and some will be done by a small tractor
from above the creek bed. Large culvert(s) will be utilized to not obstruct or hinder water Flow. No clearing or altering will be done outside of the area of
this one single lane bridge.
2. Compensatory Mitigation for Impacts to Waters of the U.S. or Waters of the State
2a. Does the project require Compensatory Mitigation for impacts to Waters of the U.S. or Waters of the State?
Yes No
2b. If this project DOES NOT require Compensatory Mitigation, explain why:
Total area affected will be less than 0.05 acres. This bridge will have minimal effect on water. This unnamed creek does not contain any fish (including trout) as it is too small/shallow.
F. Stormwater Management and Diffuse Flow Plan (required by DWR)
1. Diffuse Flow Plan
1a. Does the project include or is it adjacent to protected riparian buffers identified within one of the NC Riparian Buffer Protection Rules?
Yes No
If no, explain why:
There is no storm water drainage needed at this site.
2. Stormwater Management Plan
2a. Is this a NCDOT project subject to compliance with NCDOT's Individual NPDES permit NCS000250?
Yes No
2b. Does this project meet the requirements for low density projects as defined in 15A NCAC 02H .1003(2)?
Yes No
