received on July 17, 2023. However, additional information is required before the review may be
completed. Please address the items on the attached pages no later than the close of business on October
23, 2023.

the Cedar Point High -Rate Infiltration System. However, Mr. Blind has not been designated as a
Back -Up ORC for this facility. The Back -Up ORC currently designated for this facility is Donald
Omara. To designate Mr. Blind as the Back -Up ORC for this facility, please submit a Water
Pollution Control System Operator Designation Form (attached). It is important to note that the
Non -Discharge Branch does not process this form. Please submit the form to the Water Pollution
