Proposed Activity Provide a detailed description of the proposed activity including its purpose and include the type of equipment to be used: * The runoff from my house and my neighbor's house is concentrating and causing gully erosion to occur along the stream. I would like to perform minor filling and grading to repair the gully that has formed. The ground will be restored to spread the runoff and prevent future erosion. Thanks. D. Impact Justification and Mitigation 1. Avoidance and Minimization 1a. Specifically describe measures taken to avoid or minimize the proposed impacts in designing the project. This is naturally occurring erosion that has worsened overtime. The gully erosion is allowing sediment to be deposited into the stream. Correcting the drainage will prevent future erosion. 1b. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques. Very minimal grading and fill will be required to correct the erosion. Once filled and regraded, the area will be seeded and protected with matting. Straw bales or waddles may be needed to slow the runoff velocity until permanent groundcover is established. 6 ..................... E.
