Per our conversation yesterday, I visited the Rumley Rd bridge replacement site today and directed the workers to
remove the built up sediment in the stream on the upstream and downstream sides of the bridge (pictures attached)
utilizing the sediment removal guidelines. I am also attaching the ICA issued by our Roadside Environmental Engineer
that explains what happened and describes other measures that are to be taken concerning the sediment loss. Please

ICA / PCN Comments:
A PCN is being issued due to off-site sediment in stream channel. Jerry Parker, DEO was notified and plans to
review site on Friday. Please notify this office once the agencies have reviewed the site and made
recommendations.
Remarks and Recommendations:
Talked to Chris Fine and Jody Andrews while on the project and Chris Kirkman by telphone.
Project is out of compliance due to failure to follow approved plan and failure to take all reasonable measures
to retain sediment on site.
Project received approx. .25" of rain last night and off-site sediment was deposited into stream.
Contractor has constructed bottomless culvert and has back-filled and is installing crossline pipe at this time.
Please address the following corrective actions:
1) Install TRSC's or SSCF in corners of culvert, asap to prevent future sedimentation issues.
2) Install silt fence around headwall areas.
3) Perform maint. to remaining e. c. measures, esp. at sta.13+00 Lt.
4) Install temp. diversions/berms to divert runoff to e. c. measures in a controlled manner.
5) Install remaining e. c. measures, as shown on plans.
6) Once crossline pipe is complete, final grade, and stabilize inlet and outlet end of pipe.
7) Temp. seed/mulch disturbed areas, as needed to meet NCG01 seeding requirements.

8) Small amount of sediment was deposited in stream at both ends of culvert. Jerry Parker, DEO is scheduled
to be on site Friday morning to investigate and recommend guidance on possible removal.
NPDES documentation was reviewed. Ensure SDO's are listed on every report and all columns are filled out
completely. Continue to perForm NPDES inspection every 7 days and/or within 24 hrs. after a.5" or greater
rainfall event.
Project will be reviewed again for compliance within 5 working days.
