Additionally, as a corrective action to ensure that this situation is not repeated again we would like to propose the implementation of the Permitted Area Access Permission which would have to be signed off by Roger Mc Neil and the Head Inspector of NCDOT (Scott). This way we ensure that, ahead of time, we are planning how are we going to proceed in a permitted area and that everybody is aware of how we will proceed. I am sending you the form in Word file so that all the team can feel free to propose changes/edits and we can start implementing it ASAP. They did not issue an NOV today, but were disappointed that this could have been avoided with simple communication. Even though they did not issue and NOV, it was still a violation of the permit. NCDOT’s actions in shutting you down and getting the site protected yesterday is the main reason this was avoided yet again. 1 Tomorrow the structure crews can return to work as normal. Scott stated that there were still several items in the urgent column of the NPDES report that are still not complete. These items need to be complete before the grading/pipe installations resume tomorrow. Once the urgent items are complete, all other operations can resume and continue with the dedicated EC crew to ensure that the routine items get complete within 5 days. Before resuming work at Site 12 or entering any other permitted area a review of the plans needs to take place as well as NCDOT needs to be informed. If additional training or help with these permitted sites is necessary please advise. If I find evidence of such willful and blatant disregard for any of the permitted areas in the future as I did at Site 12 yesterday, those responsible will be removed from the job.
