Art King, DEO, has been notified and has reviewed site this afternoon. Please notify this office once stream has been restored. Remarks and Recommendations: Talked with Asst. Res. Engr., Frankie Williamson, while on site and Div. Constr. Engr., John Olinger has been notified by telephone. Project is out of compliance due to failure to take all reasonable measures to prevent off-site sedimentation. Project is a grade, base and pave operation after Randolph Co. maintenance completed a secondary rd. project. Contractor failed to take all reasonble measures by not installing weeps in channeled material stockpiled along shoulders. Runoff from a 2.5 inch storm event was then directed to permit sites # 1 and 2 and overwhelmed the silt fence and lost silt in creek at site 2 and in pipe at site 1. DEO, Art King, was notified and reviewed site after my review was completed. Art indicated that silt in pipe at permit site 1 can remain and would contribute to natural silting of bottom of pipe. Art indicated that offsite silt in creek at permit site 2 would need to be retrieved by hand. Please note the following corrective actions: 1) Install adequate number of weeps throughout project to direct runoff to basins in ditchline. There are existing measures/basins in ditches from maintenance operation that should be utilized. 2) Ensure silt fence over headwall areas of pipes, esp. permitted pipes, are well maintained and release points will direct runoff to beginning of basins. Contractor had already started this work before review was completed. 3) Would highly recommend that channeled material not be stockpiled over pipes, esp. permitted pipes. 4) Temp. s/m stockpiled material to comply with NCG01 soil stabilization requirements. 5) NPDES reviews needs to resume due to amount of distubed area.
