The Division of Water Resources (DWR) has performed a review of a 401 Water Quality Certification application received on July
17, 2015 for the above referenced project. The Division has determined that your application is incomplete and cannot be
processed. The application is on -hold until all of the following information is received:
Please provide a new Page 4 of the permit application (PCN) with a modified Table C.3 that lists ail after - the -fact
and proposed impact lengths to streams and /or wetlands on the property. The submitted application only lists
the after- the -fact impacts. [15A NCAC 02H .0502(b)(2) and (c)(2)]
Please provide a property map with the location of the proposed 36 -inch culvert. [15A NCAC 02H .0502(b)(2)]
Per Condition #13 of GC3890, ,placement of culverts and other structures in waters and streams must be below
the elevation of the streambed by one foot for all culverts with a diameter greater than 48 inches, and 20 percent
of the culvert diameter far culverts having a diameter less than or equal to 48 inches, to allow low flow passage
of water and aquatic life. Please provide a typical cross - section for the proposed culvert illustrating the
appropriate burial depth. [15A NCAC 02H .0506(b)(2)]
