The Division has determined that your application is incomplete and cannot be processed. The application is on -hold until all of the following information is received: Please provide a new Page 4 of the permit application (PCN) with a modified Table C.3 that lists ail after - the -fact and proposed impact lengths to streams and /or wetlands on the property. The submitted application only lists the after- the -fact impacts. [15A NCAC 02H .0502(b)(2) and (c)(2)] Please provide a property map with the location of the proposed 36 -inch culvert. [15A NCAC 02H .0502(b)(2)] Per Condition #13 of GC3890, ,placement of culverts and other structures in waters and streams must be below the elevation of the streambed by one foot for all culverts with a diameter greater than 48 inches, and 20 percent of the culvert diameter far culverts having a diameter less than or equal to 48 inches, to allow low flow passage of water and aquatic life. Please provide a typical cross - section for the proposed culvert illustrating the appropriate burial depth.
