Crystal Amschler, Kevin Barnett, and Mark Davis were present during the inspection. The following are items that need attention. Urgent items should be addressed immediatley. Section CA - Station 58+00 LT (Site 6) - Work area needs to be dewatered. Impervious dike is not working properly and needs to be sealed. If work is complete, then erosion control measures should be added at the top of bank to prevent exposed soils from eroding into the wet channel. URGENT - Station 134+50 RT (Site 13) Inlet - Coir fiber has been added to the channel but should completely cover the disturbed substrate directly above the invert and at the upstream end of the channel. Fill slopes above the outlet and inlet have been mulched, but should be matted. - Station 137+25 LT (Site 14) - Disturbed wetland (permitted impact) needs to be seeded and stabilized. Use native seed mix. This was called out during the last inspection. URGENT - Station 148+00 LT (Site 16) - Adjacent property owner has dredged a jurisdictional feature and placed spoils within the DOT Right of Way. Photos and a description of this were submitted to the DWR Regional Office and the Corps rep for Graham County. Site visit is pending. - Station L 163+00 LT (Site 19)- Outlet needs to be stabilized before next rain event. This was called out in the last inspection. URGENT Section CB - Station 324+00 RT (Site 19) - Disturbed area above outlet needs to be stabilized. - Station 329+50 RT (Site 21) - Mark indicated that this was on tribal land. Recommend measures at outlet to protect JS. Coordinate with tribal agency if needed. Section CC - Station Y2 78+25 RT (Site 17) - (Restoration Site) Fill voids of Class II rip rap in channel with smaller material to prevent water from piping. - Station Y2 76+25 RT (Site 17) - Add coir fiber to sloughing stream bank at confluence. - Station Y2 76+00 RT (Site 17) - Add smaller material to fill voids in rip rap outlet to prevent piping. May want to consider mashing the Class I into the channel in order to key it in better. - Station Y2 68+00 RT (Site 14) - Outlet work still needs to be completed before Jan 1 moratorium. Backfilling around headwall, rip rap at outlet and banks, and removal of rock and debris from around the stream. Plans indicate that the outlet of the 66" does not need to be buried due to slope. In -stream work and land disturbance within the 25-foot buffer zone are prohibited during the trout -spawning season of January 1 through April 15 to protect the egg and fry stages of trout. Unpermitted fill was observed in the wetland. DOT is in the process of submitting a modification to the permit to address this violation. page: 3
