Attached is the Renewal COC for NCG080455, effective 11/6/2023. The hard copy is being mailed out to Mr. Cupp this
week, and the current NCG080000 General Permit can be found on our website here. I've also attached the annual fee
invoice for this permit fee year that Tev mailed out recently, and a permit summary report with the updated outfall

Only thing I see on the form is that the Permit Contact Person is Joey but you have an old Wilco Hess address
The address should be changed to the Lonas Dr in Knoxville
Otherwise looks good
We have a cooler for this site and plan to get sample before end of year with all the others.

The permit COC has been renewed effective November 6, 2023. We are preparing the COC and other paperwork to go
out to the permittee, but in the meantime, I wanted to send the report over to verify the active status and have you
check the info in case something needs to be updated. We especially want to verify the billing contact and owner
information since we have to resume the annual fee invoices.

Stormwater Annual Permit Fee Open
This Stormwater annual fee is required by the North Carolina Administrative Code 143-215.D(a). It covers the
administrative costs associated with your permit. It is required of any person holding a permit for any time during the
annual fee period, regardless of the facility's operating status. Failure to pay the fee by the due date will subject the
permit to revocation. Operating without a valid permit is a violation and is subject to a $10,000 per day fine. If the
permit is revoked and you later decide a permit is needed, you must reapply, with the understanding the permit
request may be denied due to changes in environmental, regulatory, or modeling conditions.

for returned checks in accordance with N.C. General Statute 25-3-512.
