The Town of Fuquay-Varina inadvertently listed First Capital
Investments, LLC as the addressee on their Erosion Control Plan
Approval Letter date 3/17/2023. First Capital is a development
consultant for John Spurrell at McKee Homes LLC. Cody Simonson
at the TOFV revised his Approval Letter showing John Spurrell as
the addressee and noted a revision date for the letter as 11/7/2023. 1
didn't know which date to show in Item#1 above,so I kept the
original date of 3/17/2023 since that was when the plan was
approved.

compliant with Part II(Stormwater Pollution Prevention Plan)of the NCG010000
General Permit, I will nonetheless ensure that all conditions of Part II of the permit are
met on the project at all times.
* I hereby request coverage under the NCG010000 General Permit and understand that
coverage under this permit will constitute the permit requirements for the discharge(s)
and is enforceable in the same manner as an individual permit.
