After reviewing the paperwork, it appears the NCGO1 certificate of coverage was approved and issued with a typo on the
paperwork. The correct permit number is SEC-045063-2020. Let me know if you need anything else from me.

Good morning. I am hoping you can help me. We submitted an NCGO1 termination of coverage for 5144 Avalaire Oaks
Drive. It got denied because the SEC-045078-2020 number on our NCGO1 did not match the SEC-045063-2020 permit
number on our Certificate of Completion.

After looking at it, I think there was just a typo early on in a document because the suffix(045078-2020) if our building
permit number for this address.
I have talked to Kieu Tran at NCDEQ and she said if some from WC Enviro Services can send something saying it was a
typo, she can accept that and process our termination.
I have attached the original NCGO1,the CoC you signed, and the declination notice. Please let me know if you have any
questions.
