Thank you for your request to participate in the Streamlined Industrial User Permit Review process.You have
successfully completed the requirements for participation in the program by achieving the grade of Pretreatment
Coordinator III and submitting 3 Industrial User Permits (IUPs) for a detailed review by the Municipal Permitting
Unit. Please include the Streamlined Industrial User Permit Review Submittal Form with future permit

Your approval number,to be included on the streamlined submittal form, is 0145.
Your approval to submit permits through the streamlined permit review process will not expire. If new regulations
or language is implemented which require the permit writer to update their knowledge or skills,you will be
informed and provided with an appropriate timeframe to attend a refresher workshop.
The Division of Water Resources at NC DEQ has the authority to scrutinize any IUP submitted by an approved
streamlined permit writer. In the event of any non-compliance with 15A 2h .0917(f),you will be informed during
the review and required to rectify any issues. Please note that this review may go beyond the standard 30-day
review period.
