permit. Accordingly, we are forwarding the attached final NPDES discharge permit. This permit is
issued pursuant to the requirements of North Carolina General Statute 143-215.1. and the
Memorandum of Agreement between North Carolina and the U.S. Environmental Protection
Agency dated October 15, 2007 (or as subsequently amended).
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within
thirty (30) days following receipt of this letter. This request must be in the form of a written
petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the
Office of Administrative Hearings (6714 Mail Service Center, Raleigh, North Carolina 27699-
6714). Unless such demand is made, this permit shall be final and binding.
Please take notice that this permit is not transferable. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Quality or
permits required by the Division of Land Resources, Coastal Area Management Act, or any other
Federal or Local governmental permits which may be required.

SUPPLEMENT TO PERMIT COVER SHEET
All previous NPDES Permits issued to this facility, whether for operation or discharge are
hereby revoked. As of this permit issuance, any previously issued permit bearing this number
is no longer effective. Therefore, the exclusive authority to operate and discharge from this
facility arises under the permit conditions, requirements, terms, and provisions included herein.
Lissara Partners, LLC is hereby authorized to:
1. To operate an existing 0.054MGD wastewater treatment facility with the following

This facility is located in Forsyth County (9689 Wildflower Woods Way, Lewisville).
2. Upon receipt of an Authorization to Construct permit from the Division, construct wastewater
treatment facilities not to exceed 0.108 MGD.
3. After submitting an Engineer's Certification, discharge wastewater from said treatment works at
the location specified on the attached map into the Yadkin River which is currently classified
WS-iV waters in sub -basin 03-07-02 in the Yadkin Pee Dee River Basin.

Enclosed is our renewal for the referenced NPDES permit.
The renewal notice was sent to the former address of one of
our members and delivered to our office on February 15, 2013
after having it forwarded to his new address. First off I
would ask that you change the contact and address as follows:

Secondly, I would hope you will forgive our tardiness in
submitting this renewal because of the office movement by
Mr. Brant Godfrey, our previous contact member.
Thirdly you will see that we haven't as yet put the WWTP
into operation as the slowness of the residential buildout -
has not made this necessary to this point. It will happen
in 2013 but we are not sure exactly when that will come about.
Pending that, we have a pump -and -haul permit from DENR here
in Forsyth County that is still active. Obviously we will be
9—falharging wastewater after July 31, 2013 and must renew.

project will involve haridling approximately 90 to 50 dry tons per year as a send -solid material
with 2% to 5 % solids. These materials are suitable for land application or with some thickening
as a nutrient source for our compost operation.

Once the plant is operational, it will be monitored every day by L & N Wastewater
Management Company in Clemmons, North Carolina. When the sludge has built up to the
point where it can no longer be further degraded and needs removal, the sludge will be
transported to Granville Farms, Inc. (see attached letter) for their appropriate disposal.

Commission implementing that Article, or who falsifies, tampers with, or knowingly renders inaccurate any recording or monitoring device or method
required to be operated or maintained under Article 21 or regulations of the Environmental Management Commission implementing that Article, shall be

RESIDUALS MANAGEMENT PLAN
LISSARA WASTEWATER TREATMENT
The planned Lissara Wastewater Treatment Plant is to be an extended aeration style
activated sludge wastewater treatment plant. The developers of Lissara subdivision plan
to turn the facility over to a utility that is regulated by the NC Utilities Commission, after
the treatment plant is constructed. The utility company will then enter into a contract
with a sludge disposal company (or simply modify their existing contract), and the
sludge company will then proceed with either permitting disposal sites or selecting a
POTW that will accept the sludge from the Lissara WWTP.
This Residuals Management Plan for the plant will provide, in as much detail as is
possible at this time:
• A narrative describing the size and operation of the sludge processing train in the
proposed wastewater treatment plants
• An explanation of how the Lissara WWTP will achieve Class B treatment of the
sludge
• The estimated quantity of biosolids that will be produced by the initial and ultimate
facilities when they are operating at design capacity, and the associated storage
volume provided
Narrative: The initial treatment plant will have a design flow capacity of 54,000
gallons per day, and a sludge storage volume of 16,200 gallons. The sludge storage
volume is approximately 30% of the rated flow of the treatment plants. Each sludge
holding tank has a decanting device to allow the operator to withdraw supernatant from
the thickened sludge, before more sludge is added and the thickening process is
repeated. The supernatant is returned to the flow equalization basin.
Sludge Conditioning for Class B Standards: The facility will comply with the 40
CFR Part 503 regulations. The selected operating utility will be able to condition the
sludge to Class B standards in the sludge holding tanks by raising and maintaining the
sludge pH at 12.0 or higher for at least 2 hours, and subsequently maintaining the pH
at more than 11.5 for 22 hours. The pH conditioning would be scheduled and done
immediately prior to the sludge being pumped and removed by the contract sludge
disposal company. The pH adjustment will most likely be accomplished by use of liquid
lime, although the operating utility may elect to utilize other products to raise the pH as
required. The diffused aeration system in each tank will be used to mix tank contents.

Biosolids 4uantity and Storage: A minimum of 30 days of sludge storage at design
flow is required. The estimated quantity of sludge that will be produced at the total
rated flow of the initial treatment plant (0.054 MGD of wastewater flow) is as follows:
(0.054 mgd)(160 mg/I BOD in influent from STEP systems)(8.34 lbs./gal.) = 72 lbs.

Using a sludge yield of 0.65 lbs./lb. BOD consumed yields:
(72 lbs. BOD/day)(0.65 lbs. sludge/lb. BOD) = 47 lbs. waste sludge per day
Assume sludge holding -thickening tanks can achieve a solids concentration of at least
2.00%. The volume of the daily waste sludge, thickened to 2.00% is:

Thirty days storage of this volume would require: (282 gpd)(30 days) = 8,460 gal.
The volume of sludge storage provided is 16,200 gallons, which will theoretically
provide 57 days of storage. It is anticipated by the design engineer that the thickened
sludge concentration will average closer to 3% solids, which would provide
approximately 86 days of storage at design flow.
Land Application: The land application permitting for the sludge (if a POTW is not
used for disposal) will be accomplished by the contract sludge disposal company that is
selected by the operating utility.
A confirmation that the treatment plant has been turned over to an operating utility will
be furnished to DWQ prior to the plant going into operation. The operating utility will
then confirm to DWQ when a sludge disposal contract is in place, and that the company
has the disposal capacity required to serve the Ussara WWTP.
