Division personnel have reviewed and approved your application for a permit modification received on September 2,
2008. You requested that NPDES Permit NC0088749, issued August 15, 2008, be modified to allow the treatment
plant construction to be accomplished in phases. The requested phasing of the 0.108 MGD permitted flow will be as
follows: rj4r000
1. Phase 1--45,980 gallons per day (0.054 MGD)
2. Phase 2—108,000 gallons per day (0.108 MGD)
The Division agrees with this request and has modified the permit accordingly. We are forwarding you the modified
effluent limitations and monitoring requirement pages of your existing NPDES discharge permit which reflect this new
phased construction flow and associated effluent limits and monitoring requirements. You should replace your
existing effluent page from your current permit with these two modified effluent pages pages. This permit
modification is issued pursuant to the requirements of North Carolina General Statute 143-215.1 and the
Memorandum of Agreement between North Carolina and the U.S. Environmental Protection Agency dated October
15, 2007 (or as subsequently amended).
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to you,
you have the right to an adjudicatory hearing upon written request within thirty (30) days following receipt of this letter.
This request must be in the form of a written petition, conforming to Chapter 150B of the North Carolina General
Statutes, and filed with the Office of Administrative Hearings (6714 Mail Service Center, Raleigh, North Carolina
27699-6714). Unless such demand is made, this decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division may require
modification or revocation and reissuance of the permit. This permit does not affect the legal requirements to obtain
other permits which may be required by the Division of Water Quality or permits required by the Division of Land
Resources, the Coastal Area Management Act or any other Federal or Local governmental permit that may be

A (1). EFFLUENT LIMITATIONS AND MONITORING REQUIREMENTS
During the period beginning on the effective date of this permit and lasting until expiration or
expansion above 0.540 MGD, the Permittee is authorized to discharge treated domestic wastewater from
outfall serial number 001. Such discharges shall be limited and monitored by the Permittee as specified
below:

1. TRC limit and monitoring requirements shall apply only if chlorine or a chlorine derivative is
used for disinfection. The Division shall consider all effluent TRC values reported below 50 ug/1
to be in compliance with the permit. However, the Permittee shall continue to record and submit
all values reported by a North Carolina certified laboratory (including field certified), even if
these values fall below 50 ug/1.
There shall be no discharge of floating solids or foam visible in other than trace amounts.

treated domestic wastewater from outfall serial number 001. Such discharges shall be limited and
monitored by the Permittee as specified below:

I am requesting the referenced permit be modified to allow the required treatment plant
construction to be accomplished in phases. The requested phasing of the 0.108 MGD
permitted flow is as follows:
Phase 1 - 54,000 gallons per day (0.054 MGD)
Phase 2 - 108,000 gallons per day (0.108 MGD)
Each phase would have a treatment plant rated for 54,000 gpd.
Please contact John F. Phillips, P.E., of Diehl & Phillips, P.A., or me if you have any
questions or require any additional information. It is my understanding that there is no
fee required to process this minor modification, but please advise if this is not the case.
