Division personnel have reviewed and approved your application for renewal of the subject
permit. Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued
pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of
Agreement between North Carolina and the U.S. Environmental Protection Agency dated October 15,
2007 (or as subsequently amended).
This final permit contains no significant changes from the draft you were sent on May 21, 2007.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty
(30) days following receipt of this letter. This request must be in the form of a written petition,
conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of
Administrative Hearings (6714 Mail Service Center, Raleigh, North Carolina 27699-6714). Unless such
demand is made, this decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division
may require modification or revocation and reissuance of the permit. This permit does not affect the
legal requirements to obtain other permits which may be required by the Division of Water Quality or
pern-lits required by the Division of Land Resources, the Coastal Area Management Act or any other
Federal or Local governmental permit that may be required.

All previous NPDES Permits issued to this facility, whether for operation or discharge are hereby revoked. As of
this permit issuance, any previously issued permit bearing this number is no longer effective. Therefore, the
exclusive authority to operate and discharge from this facility arises under the permit conditions, requirements,

John, Thanks for the thoughtful reply to my comments. I especially like the fact that Davie County's
Sparks Road WTP will be an "Inform Only" number programmed into the WWTP's SCADA system.
Operators at Sparks Road WTP will need the name and number of the WWTP's ORC.
Toya, With John's reply, I do not object to the issuance of the NPDES permit, provided the facility is
operated and maintained properly, and the stated effluent limits are met before discharge. Beth Dirks or
Jeff Crisco may have additional questions.

Sorry it has taken me so long to get back to you on this one.
In view of the fact that the proposed 108,000 gpd WWTP is rather close to Davie County's Sparks Road
Water Treatment Plant, I would like for the applicant to answer the following questions:
• Will an initial low waste water loading rate create operational problems? If so, how will this be
handled?
• What type of early warning system will be employed so that Davie County's Sparks Road Water
Treatment Plant will be notified when there is a WWTP upset?
• Will there be any kind of SCADA system for automatic warning of a WWTP upset?
• What type of operational data will be collected to ensure that sludge is consistently wasted to the
sludge holding tank, and then pumped from the WWTP and taken to a proper disposal facility?
Beth, Does Davie County have any other concerns that you would like to see addressed?

Presented below are our responses to your comments and questions regarding the NDPES
permit application (your questions in italics).
Will an initial low wastewater -loading rate create operational problems? If so, how will
this be handled?
The plant would be constructed in two phases, each rated for 54,000 gallons per day. The
initial 54,000 gpd plant would consist of two parallel process trains, to meet the DWQ's
redundancy requirements. Each process train would therefore be rated for 27,000 gpd. The
initial loading on one 27,000-gpd-process train will be relatively low, as the first houses are
sold and occupied. Typically, when a plant is not supplied with a sufficient organic loading
in the start-up phase, the operator can supplement the plant influent with various products.
The possible choices range from adding bagged animal feed to the plant influent to adding
carbon source chemicals such as methanol or the methanol substitute product MicroC (two
gallons of methanol would provide a daily BOD5 loading that is equivalent to
approximately 25% of the design organic loading for one 27,000 gpd process train).
The developers of Lissara plan to turn the wastewater treatment plant over to a privately
owned utility company, such as Aqua -NC, for operation of the plant. Aqua would maintain
permit compliance for this plant's effluent during the subdivision start-up period, just as
has been done on numerous other treatment systems serving new residential subdivisions.

What type of early warning system will be employed so that Davie County's Sparks Road
Water Treatment Plant will be notified when there is a WWTP upset? Will there be any
kind of SCADA system for automatic warning of a WWTP upset?
The plant will have a SCADA/autodialer system that will notify the operator if any of the
plant equipment ceases to operate properly. The list of inputs will be finalized in the design
phase, but it would typically include a high water alarm for the now equalization basin,
notification of a loss of utility power, notification that the automatic transfer switch has
moved to the standby position (signifying the standby power generator started, came up to
proper voltage and frequency, and is providing power to the plant), a generator trouble
alarm, and a loss of air pressure when a blower is operating (indicating a motor or belt
failure). The SCADA system will telephone or page the operator regarding the
condition/alarm, and will continue to call/page until the operator acknowledges the
call/page. If the condition is not cleared within a specified time period, the system will
again call the operator until the call is acknowledged. The system will also have an
"Inform Only" list of phone numbers that it will call until the call is answered and the
system can inform the answering person (or message recorder) of the conditions/alarm
detected at the plant site. The calls made to the "Inform Only" list do not require an
acknowledgement; this feature was created for the purpose of notifying interested third
parties of the calls made to the operator without requiring the third party to enter an
acknowledgement code. The telephone for the Sparks Road WTP would be an Inform Only
number programmed into the SCADA system.
It should also be noted that Lissara subdivision is planning a Septic Tank Effluent Pump
system for each lot, and a pressure sewer to convey the septic tank effluent to the treatment
plant. The effluent from the collection system will be pumped into the plant's flow
equalization basin. Although the flow is normally pumped by the flow equalization pumps
into the plant, it is possible for the equalization basin contents to flow through the plant
without being pumped. The disinfection system is planned to be tablet chlorination and
dechlorination. Therefore, even if the plant had a catastrophic multiple failure of power
supplies, or failures of the primary and backup blowers or pumps, the wastewater that
would reach the Yadkin River would still have received treatment by primary settling and
disinfection, and would be relatively low in suspended solids. The SCADA system would
notify the operator of any of these possible malfunctions that could create upset conditions,
so that the operator could quickly respond and rectify the situation.
What type of operational data will be collected to ensure that sludge is consistently wasted
to the sludge holding tank, and then pumped from the WWTP and taken to a proper
disposal facility?
Prior to obtaining an Authorization to Construct, all NPDES permittees are required to
submit to DWQ a Residuals Management Plan. This plan must demonstrate that the plant
has sufficient sludge storage capacity, and indicate where the sludge will be finally
disposed. The operator will make the determination when to waste sludge out of the
process by checking the mixed liquor suspended solids concentration and the influent
wastewater's organic strength, and then calculating the amount of sludge that should be

wasted daily or weekly. Whether the sludge is transported to a land application site or a
publicly owned treatment plant, the party holding the sludge permit will require a written
record of the date, solids concentration, and volume of sludge hauled from the plant. The
operator will also maintain a log of all activities of the plant operation, and this log is
typically inspected during DWQ's annual Compliance Inspection of the plant.

I. Summary
Lissara Partners, LLC has proposed to build a 225-lot development on 246 acres of land in
Forsyth County. This development is expected to generate 108,000 gpd of wastewater, which
will be discharged to the Yadkin River. This segment of the Yadkin is classified WS-IV and is
listed on the draft 2008 303(d) list for turbidity standard violations.
The EAA was been approved on May 14, 2008. No level B model was performed due to the
extremely high level of dilution available instream. The facility will receive secondary limits as a
result of its 0.03% IWC. The Division reviewed all the available alternatives and discharge to the
surface water was found to be the most environmentally sound, reasonable cost effective
alternative as required by 15A NCAC 2H.0105 (c)(2).
