Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued pursuant to
the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between
North Carolina and the U.S. Environmental Protection Agency dated October 15, 2007 (or as subsequently
amended).
This final permit includes no major changes from the draft permit sent to you on April 25,
2012:
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30)
days following receipt of this letter. This request must be in the form of a written petition, conforming to
Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings
(6714 Mail Service Center, Raleigh, North Carolina 27699-6714). Unless such demand is made, this
decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division may
require modification or revocation and reissuance of the permit. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Quality or permits
required by the Division of Land Resources, the Coastal Area Management Act or any other Federal or Local
governmental permit that may be required. If you have any questions concerning this permit, please

All previous NPDES Permits, issued to this facility, whether for operation or discharge are hereby revoked. As of this
permit.issuance, any previously issued permit bearing this number is no longer effective. Therefore, the exclusive
authority to operate and discharge from this facility arises under the permit conditions, requirements, terms, and
provisions included herein.
Albert Gersing & David F. Ramsey are hereby authorized to:
1. Upon receipt of an Authorization to Construct permit from the Division, construct wastewater
treatment facilities not to exceed 0.05 MGD. This permitted facility will be located at the Stone
Bridge WWTP, off Shulls Mill Road near Boone in Watauga County.
2. After submitting an Engineer's Certification, discharge wastewater from said treatment works
at the location specified on the attached map into Lance Creek, currently classified C-Trout
waters in the Watauga River Basin.

1. U: at least 50 feet upstream from the outfall. D: at least 100 feet downstream from the outfall.
2. TRC limit and monitoring requirements apply only if chlorine is used for disinfection. The facility
shall report all effluent TRC values reported by a NC certified laboratory (including field certified
laboratories). However effluent values < 50 µg/L will be treated as zero for compliance purposes.
There shall be no discharge of floating solids or foam visible in other than trace amounts.

The NPDES permit for the Stone Bridge WWTP in Watauga County expires on September 30, 2012. This
notice is being sent to explain the requirements for your permit renewal application.
Federal (40 CFR 122) and state (15A NCAC 211.0105 (e)) regulations require that permit renewal applications
be filed at least 180 days prior to expiration of the current permit. Your renewal application is due to the
Division no later than April 3, 2012. Failure to apply for renewal by the appropriate deadline can result in a
civil penalty assessment or other enforcement activity at the discretion of the Director.
If your facility is still discharging any wastewater, this permit must be renewed. Discharge of wastewater
without a valid permit violates North Carolina General Statute 143-215.1 and could result in assessment of civil
penalties of up to $25,000 per day.
Use the attached checklist to complete your renewal package. The checklist identifies the items you must
submit with the permit renewal application. If all wastewater discharge has ceased at this facility and you wish
to rescind this permit, simply reply to this message.

Question on draft NPDES Permit NC 0036536
Dear Mr. Sledge,
The change on draft permit was adding monitoring, reporting and compliance determination of Total Residual
Chlorine valves in the event that UV disinfection system fails and chlorine compounds are used for disinfection.
I have dual UV protection so if one goes down we can valve off that one and still have disinfection. Power lose
or any other emergency the school will be closed so no usage which in turn means no discharge from
wastewater plant. So I am asking for this to be taken out of requirements in permit.
