Dear Ms: Anstead:
On February 11 and 18, 2011, Mandy Hall from the Raleigh Regional Office visited your single-family
residence (SFR) wastewater treatment system to evaluate compliance with the above permit to
discharge wastewater. Your assistance via telephone on February 15, 2011 was greatly appreciated.
The checked boxes below show what conditions were noted at your facility:
® Failure to analyze the effluent from your system once each year. See Part 1(A) of your
permit about his requirement. Make arrangements for sampling to be carried out within the
next 3 months, and submit results to this office within 3 weeks after the sampling has been
• done.
During the follow-up visit on February 18, 2011, Mrs. Hall was able to locate the discharge pipe
across the roadway from your property. At the time of the visit, the system was discharging.
The discharge was free and clear of solids.
If you have questions or comments about this inspection or the requirements to take corrective action,
please contact Mandy Hall at 919-791-4200. Licensed plumbers should be used to make plumbing
changes within your home. Contractors for installing disinfection or other equipment may be found in
the Yellow Pages under Environmental Consultants.

Gave information to family member to give to Ms. Anstead. Phone interview.
System is being maintained as required by permit. System discharges on other side of roadway, side of the
bank under brush and ivey. Would not have seen the pipe if not for the sound of the running water.
Mrs. Anstead stated that Mr. Byrd has been out to work on the system (removed trees, repaired line).
